Estudio cuasiexperimental que describe la adherencia de enfermeras hospitalarias a utilizar un modelo teórico como base de la valoración de enfermería, medida por la actitud y conducta hacia: valoración de enfermería, planificación del cuidado, conocimiento de modelos teóricos e importancia a realizar la valoración de enfermería en base a un modelo teórico. Una vez que el grupo experimental se capacitó en relación a modelos y teorías de enfemería y participó activamente en la elaboración de un instrumento de valoración, la adherencia se midió a través de las escalas de Actitud y de Conducta en Salud de Miller, adaptadas por los autores, y por la evidencia escrita en los registros. Para el análisis de los datos se usó la diferencia en diferencia mediante t de Student. Los resultados muestran que la adherencia medida por la actitud fue aceptada con una asociación estadística significativa; en cambio la adherencia medida por la conducta, no mostró diferencias significativas, resultados que fueron respaldados por la evidencia en los registros de enfermería de sólo un 11% de adherencia. Se concluye que los profesionales de enfermería, en términos de conductas adherentes, no evidenciaron cambios significativos antes y después del tratamiento experimental, pero se obtuvieron resultados positivos en términos cognitivos, ya que manifestó una actitud adherente a trabajar con un modelo teórico lo que es muy positivo para la profesión y es, por tanto, la motivación necesaria para lograr los cambios en el nuevo marco de la gestión del cuidado en enfermería.
Objective: to analyze the enablers and barriers to nurses’ professional role fulfillment during pandemics and disasters. Materials and methods: this was an integrative review based on the SALSA analytical method, carried out in eight databases, namely: Medline, Proquest, CINAHL, Web of Science, BVS, SpringerLink, Scopus, and Pubmed. The descriptors used were the following: ‘nurse’s role’ AND ‘pandemic’ AND ‘patient’ AND ‘coronavirus infection’, in English, Spanish and Portuguese. Filters along with inclusion and exclusion criteria were applied to the 11,031 articles found, after which the final sample consisted of a total of 32 articles. Results: considering the objectives set and the results obtained, it is noted that all actions focused on three main instances, namely: a) training and updating for role performance; b) predictors, enablers, and barriers for exercising the role in the face of a pandemic; and c) evidence of the role in the face of pandemics. Conclusion: this study allowed understanding both the barriers and the enablers that allow the fulfillment of nurses’ professional role in the face of pandemics or catastrophes. It also clarified that emerging infectious diseases must be addressed in an immediate, collaborative, and multidisciplinary way. Nurses’ experience, leadership, and professional training are key attributes that these professionals possess to enable their role to be more proactive during an emergency situation.
Objetivo: Determinar la efectividad de una intervención de enfermería de acompañamiento significativo estructurado para disminuir la ansiedad en pacientes que van a ser sometidos a una endoscopia digestiva alta en un hospital público, Región del Biobío, Chile. Material y Método: Ensayo clínico aleatorizado, simple ciego, con evaluación por terceros, realizado entre enero y junio de 2021, con una muestra de 126 pacientes. Los instrumentos y parámetros hemodinámicos que midieron variables sociodemográficas, psicológicas, fisiológicas} y bioquímicas fueron: Cuestionario sociodemográfico, Cuestionario de ansiedad Estado-Rasgo, Ecala Visual Análoga, Frecuencia cardiaca, Presión arterial (PA) sistólica, media y diastólica, Oximetría de pulso, Glicemia y Potasio sanguíneo. La intervención consistió en la presencia visual y táctil de un acompañante significativo, antes, durante y después del procedimiento endoscópico. El análisis de datos con SPSS v.18, consideró estadística descriptiva e inferencial (valor p 5%) con T-Student y U de Mann Whitney. Resultados: Edad promedio 52 años, educación media completa, casados y con hijos; hubo diferencias significativas en variables psicológicas (p 0,05) en las tres etapas, variables fisiológicas (PA media (p 0,0025) y diastólica (p 0,0002) intraprocedimiento; PA media (p 0,0021) y diastólica (p 0,0021) postprocedimiento, pulso medio o mediano (p 0,05) en pre (p 0,0035) y postprocedimiento endoscópico (p 0,0003). En la oximetría de pulso y las variables bioquímicas no se advirtieron diferencias significativas. Conclusión: A través del acompañamiento significativo, intervención simple, fácil, bajo costo y no farmacológica, disminuye la ansiedad y riesgo del paciente, se mejora la satisfacción usuaria y la relación equipo de salud/comunidad.
