El infarto de miocardio es una de las principales causas de muerte en el mundo, por lo que la oportunidad de un tratamiento eficaz es un reto para los sistemas de salud pública, en especial en los países en desarrollo. Describir los resultados de la trombólisis con el agente fibrinoespecífico alteplasa en pacientes con infarto de miocardio con elevación del segmento ST, en un moderado nivel de complejidad, evidenciando los beneficios de un fármaco recientemente incluido el sistema de salud colombiano, y evaluar los posibles factores asociados al resultado. Se revisaron las historias de pacientes con infarto de miocardio con elevación del segmento ST admitidos en el Hospital San Félix de La Dorada, Colombia, que cumplieron criterios para aplicación de agente fibrinolítico entre abril de 2018 y octubre de 2020, con el fin de evaluar su eficacia en la reperfusión. En un periodo de 30 meses se realizó trombólisis con alteplasa a 26 pacientes. Se cumplieron criterios de reperfusión en el 73% (n = 19) de los casos. No se presentaron complicaciones graves. No se hallaron factores asociados la presencia de criterios de reperfusión. El manejo trombolítico en pacientes con infarto de miocardio tiene una alta tasa de éxito y un bajo riesgo de complicaciones, y se puede considerar en regiones en las que se encuentra distante (> 120 min) un laboratorio de hemodinamia. Este tipo de fármacos fibrinoespecíficos deben tener disponibilidad en instituciones de moderado nivel de complejidad.
