Contexto: la enfermedad renal en etapa terminal es un problema de salud pública importante. Lograr y mantener el acceso vascular a largo plazo se vuelve difícil y desafiante en aquellos pacientes en hemodiálisis como terapia de remplazo renal. El dispositivo de injerto HeRO® (Merit Medical) ha surgido como una posible solución para pacientes con opciones limitadas para hemodiálisis. Objetivo: en el presente artículo se realiza una revisión de las generalidades, las indicaciones, la costo-efectividad, las desventajas y las potenciales complicaciones de dicho dispositivo, por medio de una búsqueda de la literatura disponible hasta la fecha. Metodología: se realizó una búsqueda sistemática en la literatura de los estudios publicados sin limitación de idioma, con el fin de obtener mayor sensibilidad y arrojar mayores resultados en la búsqueda, usando las bases de datos electrónicas de Medline, Lilacs y Europe PMC hasta el 25 de agosto del 2022. Las palabras utilizadas para la búsqueda fueron “Renal Insufficiency, Chronic”, “Dialysis”, “Renal Dialysis”, “Hero Graft access Device” y “Hero graft”. Resultados: el uso del dispositivo HeRO® tiene un futuro prometedor, ya que ofrece una forma novedosa de evitar los riesgos mortales de la dependencia prolongada del catéter. Así, se requerirá de una cuidadosa selección de pacientes, experiencia y creatividad en cirugía endovascular e intervencionismo, además de un manejo experimentado por proveedores de diálisis para promover y mantener resultados de calidad. Conclusiones: los resultados de calidad a largo plazo del injerto HeRO® definirán, en última instancia, su lugar en el ámbito del acceso vascular. El dispositivo ofrece una forma novedosa de evitar los riesgos mortales de la dependencia prolongada del catéter.
<p class="Cuerpodeltexto20">Introducción: Los ensayos clínicos controlados son el mejor diseño epidemiológico para evaluar la eficacia y seguridad de nuevas intervenciones y medicamentos. A pesar de ser estudios con asignación aleatoria, los ensayos clínicos presentan limitaciones que pueden disminuir la validez clínica de estos y la aplicabilidad en ciertos grupos de poblaciones. El objetivo del estudio es describir las limitaciones que reportan los autores de los ECA en la publicación de las 4 revistas médicas generales de alto factor de impacto.</p><p class="Cuerpodeltexto20" align="left">Metodología: Estudio de carácter observacional, descriptivo. Se escogió una muestra por conveniencia de 30 estudios en total pertenecientes al área de medicina interna y se describieron las limitaciones que expresaban los autores en la discusión del artículo. Todos los artículos pertenecientes a revistas médicas de alto factor de impacto publicados en los últimos 3 años.</p><p class="Cuerpodeltexto20" align="left">Resultados: Un total de 30 artículos de ensayos clínicos aleatorizados fueron incluidos en la revisión. Se identificaron en total 11 categorías de limitaciones diferentes; en orden de frecuencia son: atribuible a la intervención (16,3%), validez extema por población (13,1%), duración del seguimiento (13,1%), co-intervenciones (11,4%), suspensión de la intervención o retiro de los participantes durante el estudio (11,4%), medición del desenlace (9,8%), ausencia de limitaciones (9,8%), tamaño de la muestra (6,5%), identificación de los efectos adversos serios (4,9%), ausencia de grupo placebo (1,63%), y por ultimo limitación atribuible a la metodología del ensayo (1,63%).</p><p class="Cuerpodeltexto20" align="left">Conclusiones: En la serie de ECA revisados; la limitación más frecuente parte de la intervención, seguida de las que afectan la aplicabilidad clínica.</p>
