Los miembros del género Kocuria corresponden a cocos Gram positivos ubicuos, generalmente inocuos y que hacen parte de la flora saprófita de un porcentaje importante de la población; ocasionalmente han sido descritos como los agentes responsables de patologías infecciosas, principalmente dentro del contexto de pacientes que concomitantemente cursan con enfermedades crónicas y estados de inmunosupresión. Son escasos los casos reportados como causa de endocarditis en pacientes inmunocompetentes a nivel global por especies de este género. Se expone el caso de una mujer inmunocompetente de 44 años, sin antecedentesde importancia, en quien solo el diagnóstico microbiológico permitió confirmar la presencia de Kocuria kristinae como agente causal de su endocarditis infecciosa; la literatura señala la dificultad existente al momento de diferenciar la endocarditis producida por Staphylococcus spp. versus Kocuria kristinae por su evolución clínica similar, indicando la importancia de la microbiología al momento de identificar y diagnosticar apropiadamente.
IntroductionCryptococcal Meningitis is a fungal infectious disease of worldwide distribution, primarily associated with underlying immunosuppression conditions such as HIV infection, glucocorticoid treatment, status post organ transplantation and oncological treatments. Prevalence is particularly high in third-world countries where it constitutes one of the primary causes of central nervous system infections and may carry fatal outcomes. We present two cases of Cryptococcal Meningitis that portray the vast spectrum of clinical presentations associated with Cryptococcal Meningitis as well as relevant diagnostic and therapeutic implications.MethodsCase study - These adult otherwise healthy patients presented at a public urban university hospital in southern Colombia. Both had an unusual clinical course and suffered fatal outcomes despite being seemingly immunocompetent at baseline. A diagnosis of hepatic cirrhosis could have been considered a cause of immunosuppression in one of the patients and the diagnostic work-up for the other patient revealed no evidence of immunological deficiency.DiscussionCryptococcal Meningitis affecting immunocompetent individuals has been increasingly reported in recent years. Furthermore, outcomes in this population are particularly worse than those generally affected by the disease. A review of the literature related to the possible immunological mechanisms’ underlying the presented clinical course is included. We emphasize the importance of considering Cryptococcus spp. as a possible etiologic agent among differential diagnoses upon encountering suggestive meningeal conditions in immunocompetent patients.Key words: Cryptococcus neoformans, Meningitis, ImmunocompetentFunding: None.
La tuberculosis permanece como la enfermedad infecciosa que más muertes ha causado a lo largo de la historia de la existencia humana, sin poderse aún lograr su control. En años recientes ha emergido un cúmulo importante de evidencia clínica acerca de la función de la vitamina D (calcitriol o 1,25 hidroxivitamina D) en un papel que va más allá de la homeostasis del calcio. Su papel en el sistema inmunológico es muy interesante vinculando su déficit como un factor de riesgo contribuyente y así mismo fisiopatológico en un gran número de enfermedades autoinmunes e infecciosas. Dentro de éstas se destaca la tuberculosis, enfermedad endémica en nuestro país y en la cual el déficit de dicho compuesto (vitamina D) crea un factor de riesgo para desarrollar esta enfermedad en su forma activa. Presentamos una revisión detallada de los estudios que explican el papel de la vitamina D en la susceptibilidad para el desarrollo de tuberculosis y los resultados de los estudios clínicos de suplencia de la misma publicados a la fecha.
