El tromboembolismo venoso (TEV) es una enfermedad frecuente en el ámbito hospitalario, potencialmente mortal, causante de morbilidad significativa en los sobrevivientes y ampliamente prevenible. A pesar de ello, la profilaxis antitrombótica con frecuencia es subutilizada. Objetivo: establecer el grado de adherencia de los médicos de la clínica del Country (Bogotá, Colombia) a su Guía para la Prevención del Tromboembolismo Venoso en el Paciente Hospitalizado. Resultados: se analizaron 262 historias clínicas de pacientes hospitalizados entre julio 1 y 31 de 2008 y se excluyeron 22. De los 240 pacientes incluidos, 158 (65.8%) requerían profilaxis antitrombótica de acuerdo con la guía institucional. De ellos, solamente 83 (52.5%) recibió la intervención. El porcentaje de prescripción fue mayor en los pacientes de medicina interna (60%) que en los de cirugía general y especialidades (41.3%). Esta prescripción fue totalmente adecuada en cuanto a tipo de intervención, tiempo de uso y dosificación solamente en 34.5% de los pacientes. En un grupo de sujetos con riesgo tromboembólico significativo y con ausencia de la prescripción o error en la formulación de la profilaxis, las razones más importantes para ello fueron: olvido (35%), error en dosificación o tiempo de uso (27%) y subestimación del riesgo (13%). Conclusiones: la tasa de utilización de profilaxis antitrombótica en nuestra institución es similar a la reportada para el país y para el mundo, pero dista mucho de ser la deseada. Es necesario implementar medidas que modifiquen de forma eficaz la conducta médica para mejorar la prevención del TEV en los pacientes hospitalizados
Hansen disease, an emergent condition in Chile Hansen's disease (HD) is caused by Mycobacterium leprae. It has a chronic course and preferentially affects the skin and the peripheral nerves. It´s an emergent infection in Chile due to migration waves. This case report affecting a migrant worker from Haiti that presented several compatible skin lesions, with hypoesthesia and unilateral madarosis that appeared before arrival. The diagnosis of a multibacillary form was established by clinical findings, presence of fast acid bacilli on a direct skin smear, and inflammatory cell surrounding nerve endings and granulomas on skin biopsy. Besides, specific rpoB and hsp65 gene segments from M. leprae were amplified from skin samples. Patient was treated with the WHO standard combined regimen for multibacillary forms during one year showing partial regression of skin lesions. Nasopharyngeal samples showed the presence of M. leprae rpoB copies detected by PCR decreasing until six months of therapy. Notifiable diseases databases showed a recent increment of cases, all related to migrant population. Hansen´s disease is a new condition in Chile and clinicians should be aware of this possibility. Molecular tools may facilitate diagnosis and follow up.
SUMMARYThe majority (60 %) of the soils in the Venezuelan Andes are Inceptisols, a large percentage of which are classified as Dystrustepts by the US Soil Taxonomy, Second Edition of 1999. Some of these soils were classified as Humitropepts (high organic -C-OC-soils) and Dystropepts by the Soil Taxonomy prior to 1999, but no equivalent large group was created for high-OC soils in the new Ustepts suborder. Dystrusepts developed on different materials, relief and vegetation. Their properties are closely related with the parent material. Soils developed on transported deposits or sediments have darker and thicker A horizons, a slightly acid reaction, greater CEC and OC contents than upland slope soils. Based on the previous classification into large groups (Humitropepts and Dystropepts) we found that: Humitropepts have a slightly less acid and higher values of CEC than Dystropepts. These properties or characteristics seem to be related to the fact that Humitropepts have a higher clay and OC content than the Dystropepts. Canonical discrimination analysis showed that the variables that discriminate the two great soil groups from each other are OC and silt. Data for Humitropepts are grouped around the OC vector (defining axis 3, principal component analysis), while Dystropepts are associated with the clay and sand vectors, with significant correlation. Given the importance of OC for soil properties, we propose the creation of a new large group named Humustepts for the order Inceptisol, suborder Ustepts.
El tumor neuroectodérmico gastrointestinal maligno (GNET) es una entidad extremadamente rara recientemente descrita en la literatura médica y que también es conocida con el nombre de tumor similar al sarcoma de células claras en el tracto gastrointestinal (CCSLGT) por su similitud morfológica y molecular con el sarcoma de células claras de tendones y aponeurosis (CCS), pero, dada la presencia de rearreglos de gen EWSR1 y la diferenciación neuroectodérmica, se ha considerado por algunos autores más apropiado el término GNET. Es una enfermedad con pobre pronóstico, alta tasa de recurrencia y casi siempre metastásica desde el diagnóstico, lo que contrasta con el comportamiento relativamente indolente del CSS. En nuestro conocimiento, existen menos de 50 casos reportados de esta patología y a la fecha este sería el primero con primario localizado en hígado y con sobrevida libre de recaída prolongada luego de manejo quirúrgico adecuado y quimioterapia adyuvante.
