Las luxofracturas de tobillo son de las lesiones del músculo esqueléticas más frecuentes, por lo que el médico general debe estar preparado para su manejo inicial que es inmovilización y manejo del dolor; sin embargo, el empleo de imágenes diagnósticas en la mujer gestante puede resultar confuso por los riesgos a los que se puede someter a la paciente y así el abordaje terapéutico, el pronóstico y la rehabilitación pueden darse en forma tardía, generando consecuencias importantes en la funcionalidad de la articulación. En la actualidad no está establecido de forma protocolar el manejo de lesiones ortopédicas en pacientes obstétricas, por lo que resulta importante hacer una observación sobre este tema. Se presenta el caso de una paciente en primer trimestre de gestación con una luxofractura trimaleolar de tobillo, a quien se le dio manejo inicial conservador y posterior manejo quirúrgico tardío.
