ResumenIntroducción: La hernia de Spiegel es un tipo de hernia lateral de la pared abdominal poco frecuente. Una vez se diagnostica es conveniente intervenir por el alto riesgo de complicación.Caso clínico: Presentamos una paciente con antecedente de fi brilación auricular anticoagulada con apixaban. Consultó en Urgencias con signos y síntomas de obstrucción intestinal y se confi rmó la sospecha diagnóstica por tomografía de abdomen: hernia de Spiegel izquierda incarcerada. Se espero 24 horas para garantizar el consumo del anticoagulante para evitar el alto riesgo de sangrado y no 48 h, como se recomienda por el fármaco, para evitar en lo posible la evolución de la lesión y una posible perforación intestinal. El abordaje se realizó vía laparoscópica con técnica IPOM para evitar sangrados masivos por el menor trauma parietal. La paciente no presentó complicaciones. A los 12 meses permanece sin recurrencias.Discusión: Nuestro caso plantea un dilema de tratamiento médico. La paciente tiene un alto riesgo de necrosis y perforación intestinal en caso de no ser intervenida, pero también la cirugía representa un riesgo de sangrado por estar anticoagulada con apixaban (Eliquis ® ), medicamento que no tiene antídoto para revertir su efecto. Se considera que el riesgo de perforación o necrosis es mayor al de sangrado y se interviene con 24 h de latencia (no 48 h). Se decide abordaje laparoscópico porque para nosotros es el que menor riesgo de sangrado presenta. Palabras clave:Hernia de Spiegel, tratamiento, laparoscopia, anticoagulantes. AbstractIntroduction: Spiegel's hernia is a rare type of lateral hernia on the abdominal wall. Once it is diagnosed, it may be convenient to undergo surgery because of the high risk of complications.Case report: We present the case of a patient with history of atrial fi brillation anticoagulated with Apixaban. She went to the emergency department with signs and symptoms of intestinal obstruction. The diagnose was confi rmed by abdominal tomography: Incarcerated left Spiegel hernia. We decided to wait 24 hours to ensure the anticoagulant clearance, with the idea of avoiding the risk of bleeding in surgery; we considered not to wait 48 hours as recommended by the drug guidelines, avoiding as far as possible the progression of the disease and preventing a possible intestinal perforation. To prevent massive bleeding due to less parietal trauma the approach was performed laparoscopically with the IPOM technique. The patient did not present complications on surgery and 12 months later it remains without recurrence of illness.Discussion: Our case shows a dilemma of medical treatment, the patient had a high risk of necrosis and intestinal perforation in case of not being operated, but also surgery represents a risk of bleeding due to being anticoagulated with Apixaban (Eliquis ® ), a medicine that does not have antidote to reverse its effect. RESUMEN:Casseri fue un brillante anatomista y docente de la Universidad de Padua, cuya vida se vio ensombrecida por la eterna rivalidad con su mentor Fabrizio. Tras su muerte, la historia también le dio la espalda, y un cúmulo de hechos desafortunados ha ido otorgando a su discípulo Spiegel la credibilidad de buena parte de sus logros. A pesar de aportar un legado anatómico incuestionable, todavía hoy es el gran olvidado en muchos textos de la especialidad. Como tributo a su obra nosotros proponemos denominar a las hernias de la línea semilunar como Hernias de Spiegel-Casseri.. PALABRAS CLAVE:Casseri; Fabrizio; Spiegel; Anatomía; Historia. INTRODUCCIÓNGiulio Cesare Casseri nacio en Piacenza en 1552 (Fig. 1), en el seno de una familia humilde, formada por su padre Luca, sin oficio conocido, su madre Margaret y tres hermanos, Theodore, Girard y César, que murió joven. Se trasladó a Padua para estudiar medicina en calidad de sirviente de un estudiante adinerado (cliens et famulus domesticus). El destino quiso que entrara a trabajar al servicio de Fabrizio (Gerolamo Fabrici d´Acquapendente), aristócrata y anatomista de gran prestigio y fama (Riva, 2001). Fabrizio, que disponía de poco tiempo al ser continuamente reclamado por los nobles cuando estos enfermaban, dispuso de su ayudante para realizar sus disecciones y preparaciones anató-micas, y gracias a sus habilidades paso a convertirse en el asistente personal del Maestro (Fig. 2). Ingreso en la Escuela de Medicina de la Universidad Artista, doctorándose en Medicina y Filosofía en el año 1580. A partir de entonces comenzó a dar clases privadas de anatomía a estudiantes universitarios para ganarse la vida, mientras continuaba su labor de asistente de Fabrizio en su Departamento de Anatomía y Cirugía (Tomasini, 1630). En 1584, Fabrizio enfermo de artritis y tuvo que dejar temporalmente su cargo como miembro del equipo de profesores examinadores de los finales de cirugía, cargo que paso a manos de su discípulo. Por fin Casseri conseguía una holgada posición económica y solicito a su madre y a su hermano Teodoro que se trasladaran a vivir con él. Esta estabilidad le permitió concentrarse en su gran pasión, la enseñanza de la anatomía. Su entusiasmo y capacidad para la docencia le fueron reportando el agradecimiento de los estudiantes, que llegaron a solicitar formalmente en 1591, que se creara una cátedra extraordinaria de cirugía para incorporar a Casseri. En 1593, sintiendo la necesidad de compartir sus investigaciones, llamo al pintor alemán Joseph Murer para que dibujara sus disecciones (Choulant & Frank, 1962). Su vida estaba encauzada y su futuro parecía muy prometedor, pero de forma paralela al prestigio que iba consiguiendo se iba acrecentando una rivalidad con su maestro que duraría toda su vida (Bertelli, 1922;Riva).En 1598, Fabrizio decidió volver a su antiguo puesto de examinador para relegar a Casseri de ese honor.
