Resumen Entre las complicaciones de la apendicectomía podemos encontrar el dolor postoperatorio que habitualmente se relaciona con problemas locales de la herida operatoria o a más largo plazo con el desarrollo de una hernia inguinal. Presentamos un caso de un paciente apendicectomizado que presentó un dolor agudo a nivel inguinal derecho sin complicaciones locales ni evidencia de hernia u otra patología intraabdominal en las pruebas de imagen.
Resumen Introducción: los casos de endometriosis umbilical reportados tras cirugía laparoscópica ginecológica son raros, y en ocasiones suponen un auténtico reto diagnóstico. Casos clínicos: presentamos dos casos de pacientes con antecedentes de laparoscopia ginecológica (endometrioma ovárico y teratoma ovárico respectivamente) que años después presentaron a nivel umbilical un nódulo doloroso secundario a implante endometrial. Discusión: hay que distinguir entre la endometriosis umbilical primaria (nódulo de Villar) y la secundaria a implantes por procedimientos quirúrgicos. Se debe mantener un alto nivel de sospecha en este tipo de pacientes. La exéresis quirúrgica con márgenes amplios es de elección, debatiéndose la realización simultánea de una laparoscopia. Se recomienda la profilaxis mediante la extracción de la pieza operatoria en una bolsa.
Resumen El diagnóstico de la apendicitis aguda es eminentemente clínico y se apoya en estudios complementarios (analíticas y pruebas de imagen), por lo que diagnosticarla durante la realización de una colonoscopia es extraordinariamente infrecuente. Presentamos un caso de apendicitis aguda diagnosticada por colonoscopia realizada durante el control de un paciente con pólipos adenomatosos.
Resumen Introducción: la criptorquidia en pacientes ancianos es extremadamente rara y puede confundirse con una hernia inguinal complicada. Caso clínico: varón de 96 años que acudió a Urgencias por dolor inguinal izquierdo intenso con una tumoración irreductible a dicho nivel que se catalogó con episodio de incarceración. Tras la exploración física se constató que el hemiescroto izquierdo estaba vacío confirmándose mediante ecografía-doppler que se trataba de una criptorquidia izquierda que había presentado un episodio de torsión testicular. Discusión: existe poca bibliografía sobre complicaciones agudas de pacientes adultos con criptorquidia. En caso de torsión del testículo no descendido puede confundirse fácilmente con una hernia inguinal incarcerada. En algunos casos se podrá diagnosticar con una ecografía-doppler, aunque en la mayoría de las ocasiones será un hallazgo intraoperatorio.
Resumen Entre las complicaciones tardías de la cirugía gástrica podemos encontrar un amplio abanico de patologías, entre las que destaca el síndrome de obstrucción de la salida gástrica (SOSG). En los últimos 40-50 años las causas que lo producen han pasado de aquellas relacionadas con patología ulcerosa a las relacionadas con tumores malignos. Presentamos el caso de un paciente que acudió a urgencias por un cuadro de dolor abdominal asociado a un cuadro hiperemético, con antecedentes de cirugía gástrica unos 30 años antes, que presentaba un SOSG tratado mediante la colocación de un stent autoexpansible. Se revisa el diagnóstico diferencial y las diferentes opciones terapéuticas.
