Background: Nodular lymphocyte-predominant Hodgkin's lymphoma (NLPHL) is a rare subtype of Hodgkin's lymphoma showing strong CD20 expression. The role of rituximab in treating NLPHL still needs clarification. Methods: We retrospectively reviewed the outcome of 23 patients with NLPHL treated with rituximab alone or in combination with chemotherapy and/or radiotherapy as part of their first- or second-line treatment. Results: The median follow-up of the whole group was 67 months, and all patients remained alive. Twenty-two patients achieved complete remission after rituximab-based therapy, and one of them relapsed 32 months after treatment. One patient treated with rituximab alone achieved partial remission and progressed 22 months after treatment. Conclusion: The prognosis of NLPHL is excellent.Rituximab combined with chemotherapy and/or radiotherapy appears to prevent disease progression/relapse.
Introduction: Clinical trials have demonstrated the effectiveness of the CD30-targeted antibody-drug conjugate brentuximab vedotin (BV) for the treatment of relapsed/refractory Hodgkin lymphoma (R/R HL). In this study, we report on outcomes with BV in a real-world setting using data collected in clinics in the Czech Republic and Slovakia.Patients and Methods: Clinical and epidemiological data for patients with R/R HL who received treatment with BV at eight centers across the Czech Republic and Slovakia were examined. Data were amalgamated and analyzed retrospectively.Results: Clinical data for 58 patients (median age: 30.5 years) with R/R HL who received BV during the course of their treatment were collected and analyzed. Patients had received a median of 3 prior treatment regimens and most (91%) were treated with BV after relapse following autologous stem cell transplantation. Therapeutic responses after BV included 19 (33%) complete responses (CRs) and 8 (14%) partial responses. CRs occurred more frequently in patients who had received fewer prior treatment regimens. The 1-, 2-, and 3-year overall survival (OS) rates from initiation of BV were 78%, 62%, and 41%, respectively.Conclusion: Response rates and OS in this analysis of BV in real-world settings in the Czech Republic and Slovakia were consistent with those reported for pivotal clinical trials and from previous studies outside the clinical trial setting. The results support the efficacy of BV for treatment of R/R HL in real-life clinical practice.
Bloom syndrome is an autosomal recessive disorder characterized by prenatal and postnatal growth deficiency, photosensitive skin changes, immune deficiency, insulin resistance, and a greatly increased risk of early-onset cancer and development of multiple malignancies. Loss-of-function variants of the BLM gene, which codes for a RecQ helicase, cause Bloom syndrome. We report a consanguineous family, with 2 siblings showing clinical signs of suspected chromosome breakage disorder. One of them developed recurrent malignant lymphoma during lifetime. We performed next-generation sequencing analysis, focusing on cancer predisposition syndromes. We identified a homozygous pathogenic nonsense variant c.1642C>T (p.Gln548*) in the BLM gene in the proband, associated with Bloom syndrome. Sanger sequencing validated the presence of a homozygous pathogenic variant in the proband and also in the brother with short stature. In this article, we will focus on the clinical presentation of the syndrome in this particular family as well as the characteristics of malignancies found in the proband.
2 IV. interní hematologická klinika LF UK a FN Hradec Králové 3 Interní hematologická a onkologická klinika LF MU a FN Brno 4 DSC Services, s. r. o., Tišnov Souhrn Východiska: Relapsy po léčbě Hodgkinova lymfomu (HL) se vyskytují u 20-30 % pacientů. U relabovaných/ refrakterních pacientů s HL, kteří nejsou vhodní k autologní transplantaci kmenových buněk (autologous stem cell transplantation -ASCT), není určena standardní strategie léčby. V článku je retrospektivně hodnoceno přežití a prognóza pacientů s relabovaným/ refrakterním HL, kteří jsou nevhodní k vysokodávkované chemoterapii s ASCT. V diskuzi je uveden přehled nových léků používaných v této indikaci. Pacienti a metody: Do analýzy bylo zařazeno 17 pacientů, kteří byli léčeni minimálně třemi liniemi standardní chemoterapie ± radioterapie. Vysokodávkovaná chemoterapie s ASCT nebyla indikována ve 2. a další linii léčby z důvodu vysokého věku (sedm pacientů), refrakternosti HL (sedm pacientů), kardiotoxicity (dva pacienti) a pro nedostatečný štěp kmenových buněk (jeden pacient). Výsledky: Medián sledování celé skupiny pacientů od dia gnózy byl 3,48 roku. Celková odpověď na 2. linii léčby byla dosažena u osmi pacientů (47,0 %). Po 1. linii léčby byli čtyři pacienti (23,5 %) primárně refrakterní a po 2. linii léčby stoupl počet refrakterních případů o tři pacienty (17,6 %). Ze 17 pacientů žijí čtyři v remisi (23,5 %) a 13 zemřelo (osm na progresi HL, čtyři na toxicitu a u jednoho pacienta je příčina úmrtí neznámá). Pětileté celkové přežití celé skupiny od dia gnózy bylo 46,3 % a od dia gnózy prvního relapsu 30,8 %. Pětileté celkové přežití u čtyř primárně refrakterních pacientů bylo významně horší ve srovnání se skupinou 13 relabovaných pacientů: 0 vs 60,6 %, p < 0,001. Závěr: Prognóza relabovaných/ refrakterních pacientů s HL nevhodných k ASCT, kteří byli opakovaně léčeni standardní chemoterapií ± radioterapií, je špatná. U primárně refrakterních pacientů je indikováno zařazení brentuximab vedotinu do 2. linie léčby a u ostatních relabovaných pacientů do 3. linie léčby. Tato strategie pomůže zvýšit podíl remisí s cílem prodloužení celkového přežití.
