O câncer corresponde a um conjunto de doenças malignas que caracterizam-se pela multiplicação desordenada de células capazes de disseminar e invadir tecidos e órgãos. Dentre esse grupo de neoplasias, enquadram-se aqueles que acometem o trato aerodigestivo superior, sendo denominados de tumores em cabeça e pescoço. A presente revisão busca descrever as principais complicações imediatas e tardias inerentes ao tratamento radioterápico em cabeça e pescoço. Trata-se de uma revisão de literatura do tipo narrativa nas bases de dados da PubMed e BVS (Biblioteca Virtual em Saúde). Para a busca de artigos foram utilizados os descritores “Neoplasia oral”, “Radioterapia localizada”, “complicações” e cruzados através do operador booleano AND. Dentre as complicações imediatas ao tratamento radioterápico em cabeça e pescoço pode-se citar as mucosites, infecções fúngicas, disgeusia, trismo e xerostomia. Em contrapartida, outras alterações podem ser presentes com o decorrer do tempo, como a cárie de radiação e a osteorradionecrose (ORNM). A prevenção do câncer é uma tarefa fundamental, contudo, uma vez instalado, é de extrema importância o início da terapêutica antineoplásica o quanto antes. Inerente a este tratamento, o cirurgião-dentista é um profissional indispensável para o acompanhamento das ocorrências em cabeça e pescoço.
A adaptação marginal é um fator extremamente relevante na determinação da longevidade das restaurações dentárias. A vedação inadequada pode levar a inúmeras consequências. Sendo assim, o presente trabalho busca avaliar a integridade das margens de restaurações mésio-ocluso-distais em resina composta após envelhecimento por ciclagem térmica através da tomografia por coerência óptica (OCT). Foram confeccionadas cavidades mésio-ocluso-distais em 30 pré-molares humanos extraídos, distribuídos em 3 grupos para restauração em resina composta: Zirconfill (Technew), Luna (SDI) e Aura Bulk Fill (SDI). Após 24h de armazenamento em água destilada, foi realizado acabamento nas restaurações, polimento e seguiram mais 24h na água. A ciclagem foi realizada com 2.000 ciclos de imersão em água com temperatura de 5°C (± 3°C) e 55°C (± 3°C), por 15 segundos cada. Os espécimes foram analisados em OCT (OQ Labscope, Lumedica), com obtenção de imagens a cada 250 m, antes e após a ciclagem. Todos os grupos sofreram aumento e agravamento das falhas de interface dente/restauração após o estresse térmico; esses dados foram confirmados pelo teste de Kruskal-Wallis para os grupos Zirconfill e Aura Bulk Fill (p<0,0001), e pelo teste ANOVA para o grupo Luna (p<0,0001). Todavia, o grupo Aura Bulk Fill apresentou melhor desempenho quando comparada a frequência de defeitos nas margens restauradoras antes e após ciclagem. A ciclagem térmica foi determinante para o surgimento/agravamento de falhas nas margens restauradas. A Aura Bulk Fill apresentou melhor desempenho.
