Análise comparativa da complacência do sistema respiratório em três diferentes posições no leito (lateral, sentada e dorsal) em pacientes submetidos à ventilação mecânica invasiva prolongada
A fisioterapia respiratória tem papel fundamental nos casos de complicações pelo excesso de secreção broncopulmonar. Manobras de remoção de secreção brônquica como a vibrocompressão e o aumento do fluxo expiratório (AFE) garantem a perviabilidade das vias aéreas. O objetivo deste trabalho foi avaliar o efeito das manobras de AFE e vibrocompressão para remoção de secreção e suas repercussões hemodinâmicas e ventilatórias em pacientes traqueostomizados. Participaram 20 pacientes (com 18 a 73 anos) de ambos os sexos com diagnóstico fisioterapêutico de hipersecreção e/ou retenção de muco brônquico, submetidos à aplicação dessas técnicas e à remoção de secreção, que foram avaliados antes e depois da intervenção quanto a parâmetros hemodinâmicos e respiratórios. Com a aplicação da técnica de vibrocompressão foi verificada queda significativa (p<0,05) nas pressões arteriais (PA) diastólica e média de 82,2±13,4 para 77,4±13,3 mmHg e de 104,3±18,2 para 90,9±9,07 mmHg, respectivamente; o mesmo ocorreu com a PA média (p<0,05) após a aplicação da técnica de AFE, de 103,5±13,2 para 94,1±9,0 mmHg. Não houve diferença entre as técnicas aplicadas quanto ao volume de secreção removida (p=0,60). Pacientes traqueostomizados apresentaram pois redução da pressão arterial após 30 minutos da aplicação das técnicas fisioterapêuticas, mas não houve diferença entre as técnicas utilizadas quanto ao volume de secreção removida.
Estilo de vida corresponde aos hábitos, que refletem atitudes, valores e oportunidades das pessoas. As horas de trabalho diariamente podem interferir no estilo de vida saudável. Avaliou-se, transversalmente, o estilo de vida de 82 jovens estudantes, do período noturno, moradores da Zona Sul da cidade de São Paulo, correlacionando com a jornada de trabalho. Aplicaram-se os questionários: Estilo de Vida Fantástico e Cálculo da Idade Biológica e Cronológica. Calculados a média e o desvio padrão, utilizados os testes: normalidade de Kolmogorov-Smirnov e de Pearson, e analisada a variância, considerou-se p<0,05 estatisticamente signifi cante. Os estudantes declararam ter a idade de 26 ± 5,6 anos, 16 eram casados, 65%, 25% e 10% declaram ter tripla, dupla e uma única jornada de trabalho, respectivamente. A média diária de sono foi de 5,5 horas nos de tripla, 6,8 horas (p=0,004) para os de dupla e 7,1 horas (p=0,0012) nos de única jornada de trabalho. Os de dupla ou tripla jornada de trabalho apresentaram pior estilo de vida (p=0,0021) (p<0,0001), quando comparados aos de uma única jornada.
Background and objective:: Reduction of exercise tolerance is associated with expiratory flow-limitation (EFL) and lung hyperinflation; those are only partially reversible to bronchodilator. Lung hyperinflation lowers the diaphragm muscle provoking a mechanical disadvantage that, eventually, reduces maximal inspiratory (MIP) and expiratory (MEP) pressures. We aimed to assess the influence of the dynamic lung hyperinflation on respiratory pressures changes at rest and after a submaximal exercise test in COPD patients with and without bronchodilator. Methods:: We prospectively analyzed 16 COPD patients (FEV1 36.4±10% pred.; age 61.0±8 years, height 165±12 cm and BMI 25.9±6 kg/m2). MIP and MEP were measured before and after performing the six minutes walking test (6MWT) with and without bronchodilator (400 mcg of albuterol). Results:: Nine of 16 patients increased IC more than 150 ml after bronchodilator use. Right after the 6MWT accomplished without bronchodilator IC decreased 7.05% compared to the 6MWT baseline value (p<0.01). Nine patients decreased IC more than 150ml. After bronchodilator use patients performed the 6MWT without any IC significant reduction (p>0.05). Twelve patients increased the MIP (ranging from 70±11cmH2O to 77±10cmH2O, p = 0.0043) using 400mcg of albuterol. Thirteen patients reduced MIP after the 6MWT without bronchodilator use (p <0.007). There was no significant reduction (p> 0.05) in MIP when patients performed the 6MWT after bronchodilator use. We also found a significant correlation between MIP and inspiratory capacity (IC) and MEP and the IC before and after the 6MWT (r=0.61, p=0.0054; r=0.60, p=0.0031, respectively). Conclusions:: Dynamic pulmonary hyperinflation directly interferes with the ability of respiratory muscles to generate inspiratory and expiratory pressures. The previous use of bronchodilator in patients with COPD reduced dynamic hyperinflation when accomplishing a sub-maximal exercise.
Objetivo: Verificar a mortalidade, analisar as variáveis preditoras de mortalidade dos pacientes hospitalizados na unidade de terapia intensiva (UTI) por traumatismo crânio encefálico e observar a evolução dos mesmos. Método: Os dados foram coletados num hospital público de São Paulo na unidade de terapia intensiva e consistiu em avaliar o paciente diariamente e colher dados em seu prontuário. Foram estudados 20 pacientes, com idade média de 52±19,9 e 30±12 anos para o grupo óbito e vivos, respectivamente. Resultados: Não foi encontrada diferença significativa para os dois grupos na análise dos exames laboratoriais de sódio, potássio, cálcio, magnésio, uréia, creatinina, e na avaliação pela escala de Glasgow (p=0,22). Houve maior risco de mortalidade para a presença do hematoma 2,62 (IC 1,13-6,09), infecção 3,5 (IC 1,2-5,9) dos níveis séricos de leucócitos no inicio, meio e fim para os dois grupos. Para óbito e vivos não foi encontrado diferença estatisticamente significante (p=0,06). Na avaliação no qual foi comparado o leucograma inicial, meio e final para o grupo óbito separadamente, verificou-se que houve aumento significante do número de leucócitos (p=0,0011). Conclusão: A infecção e a presença do hematoma aumentaram o risco de mortalidade, e a escala de Glasgow não foi um bom preditor de mortalidade.
