A contratura de Dupuytren (CD) é uma deformidade comum de mão que progressivamente causa flexão irreversível dos dedos. É uma condição autossômica dominante de penetrância varíavel, com maior incidência no sexo masculino. O diagnóstico é clínico, baseado no exame físico das mãos. Na fase inicial, é perceptível a presença de um nódulo na região palmar, mais frequentemente em dedo anelar ou médio. Com o avanço da patologia, um cordão superficial contrai as articulações metacarpofalangianas e interfalangianas dos dedos, que por fim provoca o arqueamento da mão do paciente. O manejo ideal ainda é bastante discutido e as opções terapêuticas são variadas. O presente estudo visa revisar e contrapor as opções terapêuticas disponíveis para o tratamento da CD, avaliando os resultados e desafios clínicos. Trata-se de uma Revisão Integrativa e qualitativa realizada através de pesquisas nos bancos de dados da Scientific Electronic Library Online (SciElo), Biblioteca Virtual em Saúde (BVS), UpToDate e Medical Literature Analysis and Retrieval System Online (MEDLINE), nas línguas inglesa, portuguesa e espanhola nos últimos 5 anos. A CD é uma fisiopatologia controversa, possuindo divergências quanto ao manejo terapêutico. Dos artigos pesquisados, 42,9% (n=3) relataram o uso da Colegenase Clostridium Histolycium (CCH) com resultados positivos, como a redução do tamanho e dureza do nódulo palmar, e menor quantidade de efeitos colaterais e complicações em comparação com a fasciectomia aberta. Contudo, outro estudo 14,3% (n=1) comparou o CCH com a fasciotomia por agulha percutânea (FNP), e demonstrou nenhuma vantagem adicional. Dos artigos, 14,3% (n=1) utilizou a fração vascular do estroma a partir da digestão enzimática e centrifugação, sendo vantajoso quanto a troficidade, vascularização e regeneração dos tecidos. Além disso, 28,6% dos artigos (n=2) abordaram o ensaio clínico randomizado em fase 2a, que possui como enfoque o uso da injeção intranodular com inibidor de fator de necrose tumoral. O uso desse inibidor (40 mg de adalimumabe em 0,4 ml) resultou na redução da expressão da proteína alfa SMA, com consequente redução dos fenótipos de miofibroblastos, constituindo-se como opção terapêutica para regressão da CD. Outros tipos de tratamentos descritos na literatura são: o uso de corticoides, imobilização, fisioterapia, vitamina E e radioterapia. Ademais, para os casos mais graves são mencionados tratamentos cirúrgicos -fasciectomia total ou parcial, dermofasciectomia ou fasciotomia percutânea. Portanto, foi possível perceber que mesmo com as possibilidades comprovadas para o adequado manejo da CD, a terapêutica ainda é incerta precisando de mais estudos para consolidar a escolha, sendo necessário optar conforme a gravidade e de acordo com o grau e variedade de manifestações apresentadas.
