La tiroiditis subaguda o de Quervain, es una entidad inflamatoria de la tiroides que se caracteriza por su curso trifásico que incluye una fase de liberación excesiva de hormona tiroidea. Esta entidad suele tener un curso autolimitado y su manejo es principalmente sintomático. En este artículo se presenta el caso de un paciente masculino de 32 años de edad sin antecedentes personales patológicos, con un cuadro de 10 días de evolución de dolor en cuello anterior y síntomas compatibles con tirotoxicosis. Posteriormente, se confirmó la elevación de niveles de hormona tiroidea en sangre y se decidió manejo conservador con tratamiento sintomático. A los 12 meses de seguimiento el paciente se presenta con función tiroidea normal y completa resolución de los síntomas.
Las fracturas trimaleolares representan el 7%-11% de las fracturas de tobillo y generalmente son causadas por traumas de alta energía. Son fracturas inestables que requieren reparación quirúrgica en la mayoría de los casos pues se asocia con un mejor resultado funcional por lo que se debe considerar en todos los pacientes, incluso en aquellos con comorbilidades. En este tipo de fracturas el compromiso del maléolo posterior es el componente de mayor complejidad y se asocia a un peor pronóstico. El manejo de este tipo de fracturas aún es controversial, principalmente en cuanto al orden de la reparación de los maléolos y en los materiales de fijación que deben utilizarse.
El hipotiroidismo es el síndrome clínico caracterizado por la deficiencia de hormona tiroidea en el organismo. Este provoca cambios en el tejido conectivo, que se manifiestan como aumento de volumen y edema en distintas partes del cuerpo. Las alteraciones en el tejido conectivo pueden provocar síndromes de compresión nerviosa en el paciente hipotiroideo, siendo el síndrome de túnel carpiano el más frecuentemente asociado. En el presente artículo se relata el caso de un paciente que consulta por parestesias en miembro superior derecho y lumbalgia de características neuropáticas que llevaron al diagnóstico de síndrome del túnel carpal y neuropatía ciática, respectivamente. Se realizaron exámenes de laboratorio que documentaron hipotiroidismo severo, por lo que se inició tratamiento con levotiroxina. Seis semanas después, el paciente presenta resolución completa de las parestesias en miembro superior derecho y de la lumbalgia, lo que llevó finalmente al diagnóstico de síndromes de compresión nerviosa como manifestación del hipotiroidismo.
La enfermedad de Behçet (EB) es una vasculitis inflamatoria multisistémica de etiología incierta e infrecuente, clasificada como una enfermedad rara en Estados Unidos y países europeos. El síndrome de Neuro-Behçet (SNB) ocurre cuando se presenta compromiso neurológico en pacientes con EB, en los cuales se ha descartado otras etiologías neurológicas. El espectro de afectación neurológica en la EB es muy amplio, sin embargo, las manifestaciones clínicas y los hallazgos de neuroimagen guían hacia dos grandes grupos de SNB: parenquimatoso y no parenquimatoso. La EB se debe incluir en el diagnóstico diferencial de enfermedades inflamatorias, vasculares y desmielinizantes del SNC.
El síndrome de Cushing es el resultado de la exposición a una cantidad excesiva de glucocorticoides por un largo periodo de tiempo. Sus manifestaciones clínicas rondan desde hallazgos muy característicos como las facies de luna llena, el acúmulo de grasa dorsocervical o las estrías purpúreas; hasta presentaciones poco específicas que pueden pasar desapercibidas y llevar a diagnósticos erróneos. Dentro de los casos más retadores se encuentran aquellos cuya presentación clínica se basa en manifestaciones psiquiátricas y síntomas inespecíficos; sin embargo, es esencial tener este síndrome en mente como diagnóstico diferencial con el fin de captar la mayor cantidad de casos de forma temprana y ofrecerles el tratamiento adecuado. Se presenta un caso de una mujer de 62 años quien consulta por una segunda opinión tras un diagnóstico de depresión sin respuesta al tratamiento, en quien por medio de examen físico se logra orientar el diagnóstico hacia un síndrome de cushing endógeno primario.
