The hepatitis C virus (HCV) is classified into six different genotypes and their distribution is different throughout the world. Epidemiologic studies are important to determine several characteristics of the virus, as well as the disease. This study analysed the prevalence of HCV and its genotypes among patients from a leading hospital in Ceará, which is located in Northeast Brazil. A total of 119 anti-HCV-seropositive patients, each having previously completed a questionnaire about risk behaviours related to HCV infection were tested for HCV infection using a qualitative HCV polymerase chain reaction (PCR) assay and genotyping by restriction fragment length polymorphism (RFLP). The detection was based on amplifying of the non-coding 5' region. Of the 119 patients, 95 showed positive results in the qualitative HCV test. History of surgery was the most reported risk factor, followed by the use of drugs, having tattoos, undergoing haemodialysis and occupational exposure. Genotype 1 was the most prevalent (46.9%), followed by genotype 3 (34.4%) and 2 (8.3%). The genotype distribution was similar for all of the various risk behaviours.
OBJETIVO: Relatar a experiência pessoal no tratamento cirúrgico de hérnias incisionais com a utilização da malha de polipropileno monofilamentar-márlex. MÉTODOS: Foram operados e seguidos 74 pacientes portadores de hérnia incisional originada de tratamento cirúrgico de diferentes afecções da cavidde abdominal, com idade entre 30 e 94 anos, sendo 39 do sexo feminino e 35 do sexo masculino. Destes 41,9% foram submetidos a hernioplastia incisional pela primeira vez e 58,1% já haviam tentado o tratamento sem sucesso. A técnica operatória usada foi a dissecção do saco herniário, ressecção da fibrose resultante de operações anteriores e fixação de tela de márlex substituindo ou reforçando a fáscia transversal por baixo dos músculos da parede abdominal. RESULTADO: 74 pacientes foram operados e seguidos de 1975 a 1995. Na última revisão, em maio de 1998, três pacientes haviam falecido, dois de doenças cardíacas e um de doença neoplasia. Apenas um paciente (1,3%) apresentou recidiva, no início da experiência. Este foi reoperado e terve sua hérnia incisional curada. CONCLUSÃO: o uso da tela de márlex tecnicamente aplicada é o método ideal para a cura definitiva das hérnias incisionais.
O autor fez revisão da literatura, o que lhe permitiu afirmar que o tratamento cirúrgico das hérnias inguinais contribuiu para o desenvolvimento da cirurgia como um todo. Somente no final do século passado foi alcançado o ponto importante do progresso das técnicas operatórias. Um "fantasma continuou rondando" os pacientes operados de hérnia: a recidiva. Vários grupos de cirurgiões já demonstraram essa preocupação e, a partir de 1913, passou-se a pensar em colocação de prótese, visando substituir a fáscia transversal desgastada, nas hérnias recidivadas. Dentre os vários tipos de material de prótese, a malha de polipropileno monofilamentar - marlex®, - tomou-se o mais aceitável. Nesse trabalho propõe-se a modificação da técnica de Mc Vay pelo uso da malha de polipropileno monofilamentar marlex®, que, em 109 hernioplastias para hérnias recidivadas, seguidas durante vinte anos, resultou em recidiva zero.
