Pseudoaneurysms are rare lesions secondary to blunt or penetrating trauma, temporomandibular joint surgery, or orthognathic surgery. Nonsurgical interventions are the treatment of choice for pseudoaneurysms. In the case reported here, endovascular injection of acrylic glue was successful in the treatment of a pseudoaneurysm of the internal maxillary artery secondary to fracture of the zygomatic bone.
Trauma is one of the leading causes of death worldwide. Due to its anatomy and position in the facial skeleton, the orbit becomes a region susceptible to trauma which may also involve the penetration of foreign bodies (FBs). These events can have serious repercussions depending on their extent due to the proximity of the orbit with other noble structures of the face and skull. Because of this, a system of prehospital management of traumas must be established, in order to promote a better prognosis for patients. The present study aims to report the case of a patient suffering from a motorcycle accident with multiple fragments of FBs in the region of orbit-zygomatic-maxillary complex. The emergency surgical removal of the fragments was performed by a multidisciplinary team, involving Ophthalmology and Oral and Maxillofacial Surgery and Traumatology (OMST). Wound cleansing and debridement were performed with subsequent removal of the foreign bodies by the OMST team, in addition to the reduction and fixation of related fractures. As for Ophthalmology, the evisceration of the affected eye was performed due to its anatomical and functional impairment. The patient is 12 months postoperatively, with no aesthetic or functional complaints regarding OMST. The anatomical knowledge of the traumatized region, besides the establishment of the conduct regarding the removal of possible associated foreign bodies are required so that the success of the treatment can be obtained, aiming to minimize the damages to the patient.
The Aneurysmal Bone Cyst (ABC) is an expansive osteolytic, radiolucent, multilocular, eccentric lesion of unknown origin consisting of spaces filled with blood in any segment of the skeleton, being most frequently encountered in the diaphysis of the long bones or the spinal cord. It may occur as distinct clinico-pathological entity or as pathophysiological alteration of a preexisting lesion, like giant-cells lesion, fibrous dysplasia, ossifying fibroma, osteoblastoma, epiphysary chondroblastoma and even malignant lesions such as osteossarcoma. This paper reports a rare case of ABC in the zygomatic arch of a young female patient who presented facial asymmetry and progressive limitation in mouth opening, treated by resective surgical procedure and discusses the clinical, radiological and histological features of this pathology along with its treatment.
Introduction: Naso-orbital-ethmoidal fractures are complex fractures involving the bones of the nose, orbit region, maxilla and ethmoid. This complex structure has a close relationship with the brain and the orbital region, which frequently suffer injuries during trauma. Among traumatic brain injuries, there is the pneumoencephalon. The present study aims to report the treatment of a patient who suffered an accident involving a rural animal. Case Report: A 63-year-old male patient was brought to the highly complex hospital for the Oral and Maxillofacial Surgery and Traumatology service, five days after the trauma. The physical examination showed traumatic telecanthus,anisocoria and mydriasis of the right eye, right eyelid ptosis, laceration in the right eyebrow and deviation of the nasal bone to the left. In the imaging exam (computed tomography), traces of fractures were observed in the nasal bone region, medial orbital wall, infra-orbital margin, frontal-zygomatic pillar and right orbital blow out fracture. After a surgical approach to fix the fractures, it was observed the development of a hypodense area in the intracranial region, suggestive of pneumocephalus. Conclusion: These complications, such as pneumocephalus, must be diagnosed early and appropriate treatment must be started as soon as possible to reduce possible sequelae, requiring the intervention of a multidisciplinary team.
