La extirpación quirúrgica con márgenes suficientes continúa siendo el tratamiento de elección de estos tumores. El papel de la radioterapia es aún controvertido en los tumores desmoides, ya que persiste la discusión de si administrarla tras intervención quirúrgica con borde afecto o bien solo en los recidivantes. La radioterapia puede ser de gran valor en aquellas situaciones de inoperabilidad, bien por la extensión e infiltración tumoral, bien por contraindicación anestésica de cualquier motivo. En los sarcomas de pared, la radioterapia tiene su indicación en los extirpados con borde afecto (tamaño mayor de 5 cm) o en aquellos con un grado de diferenciación moderado o alto. En las metástasis situadas en los lugares de entrada de trocares de laparoscopia el tratamiento es quirúrgico, y la radioterapia se emplea en caso de bordes próximos o afectos. En los dermatofibrosarcomas protuberans, el tratamiento quirúrgico es el de elección, y pueden beneficiarse de radioterapia los casos con bordes afectos no subsidiarios de más intervenciones por contraindicación médica.
