La reparación de la hernia ventral (HV) con separación de componentes (SC) representa a un grupo de técnicas quirúrgicas diferentes en su concepción, desarrolladas para pacientes que requieren una reconstrucción importante de la pared abdominal. Los métodos descritos comparten elementos clave, como son la liberación del aponeurótico con avance miofascial, la reconstrucción de la línea alba y la creación de vastos espacios dentro de la pared abdominal que aseguren una amplia disposición y fijación de la malla. La diferencia entre la separación de componentes anterior (SCA) y la separación de componentes posterior (SCP) es la creación y localización de estos espacios. Los métodos de SCP permiten la colocación de una malla retromuscular, lo que la diferencia favorablemente en la bibliografía por las tasas de complicaciones de herida y de recurrencia. La SCP con liberación de transversos tiene diferentes ventajas frente a la SCA: permite una disección lateral extensa en el espacio virtual avascular por debajo del músculo transverso del abdomen, con lo que se crea de esta forma el espacio con la ubicación adecuada e ideal para implantar la malla, preservando al mismo tiempo el aporte neurovascular completo a la pared abdominal anterior. La liberación del músculo transverso del abdomen es la que permitirá medializar ampliamente los músculos rectos y la reconstrucción de las capas de la pared abdominal posterior y anterior con la malla, quedando esta en un «bolsillo» retromuscular bien vascularizado y alejado de la piel. Además, dicha posición retromuscular de la malla permitirá una amplia sutura en los defectos difíciles (subxifoideo, subcostal y suprapúbico). Basado en estas ventajas y en la baja tasa de recurrencia reportada (3-5 %), la SCP con liberación del músculo transverso abdominal es el método de reparación pretendidamente preferido para el manejo de pacientes que requieran una reparación de las HV de gran tamaño.
Hepatic mucinous cystic neoplasms represent a challenging pathology due to their very low frequency, insidious presentation and unclear demography due to the limited cases reported to date. Surgical treatment is mandatory when possible to prevent recurrence and malignant transformation. The case presented herein has a peculiarity, mainly an increased level of CA 19-9 which has not been previously reported.
