RESUMENObjetivo. Evaluar las puntuaciones de la escala de Alvarado en una serie de casos intervenidos quirúrgicamente con diagnóstico de apendicitis aguda.Materiales y métodos. Estudio descriptivo de corte transversal. Se revisaron historias de 159 pacientes sometidos a cirugía con diagnóstico de apendicitis aguda, de enero a junio de 2015. Se estimó la puntuación alcanzada en la escala de Alvarado y se correlacionó con el resultado histopatológico, tiempo de evolución, sexo y edad.Resultados. El 56,6 % correspondió a hombres y el promedio de edad de 38,4 años. El tiempo de evolución registró un promedio de 36 horas para las mujeres y 40,75 horas para los hombres. En el 94% de los casos presentaban dolor en fosa ilíaca derecha, 48% fiebre graduada, 77% náuseas o vómitos, 96% signo de Blumberg positivo, 81% anorexia, 77% migración del dolor, 78,8% leucocitosis, 88,4% neutrofilia. La media registrada con la escala de Alvarado fue de 8; 7,89 en mujeres y 7,91 en hombres. Hubo concordancia diagnóstica del 98% entre la escala de Alvarado y resultado histopatológico.Conclusión. La escala de Alvarado puede ser una herramienta útil para mejorar la eficiencia diagnóstica en la apendicitis aguda, sobre todo en los servicios de atención ambulatoria y urgencias.Palabras Claves: Apendicitis aguda, Escala diagnóstica de Alvarado, Abdomen agudo quirúrgico. SUMMARY Objective:Evaluate the scale scores of Alvarado in a series of cases operated on with diagnosis of acute appendicitis.Materials and methods. A descriptive cross-sectional study. 159 stories were reviewed of patients undergoing surgery with a diagnosis of acute appendicitis, from January to June 2015. It was felt the score attained in the scale of Alvarado and correlated with the histopathological result, time of evolution, sex and age.Results. The 56.6 % were men and the average age of 38.4 years. The time of evolution, an average of 36 hours for 40.75 hours for women and men. In 94% of cases presented pain in the right iliac fossa, 48% 77% graduate fever, nausea or vomiting, 96% positive sign of Blumberg, 81% anorexia, 77% migration of pain, 78.8% leukocytosis, 88.4% neutrophilia. The average recorded with the scale of Alvarado was of 8; 7.91 7.89 in women and in men. There were 98% of the diagnostic concordance between the scale of Alvarado and histopathological result.Conclusion. Reporte de caso RESUMEN Las duplicaciones intestinales son anomalías congénitas; se han descrito a lo largo de todo el tracto digestivo con una presentación clínica inespecífica. Reportamos el caso clínico de un paciente de 83 años intervenido quirúrgicamente por una oclusión intestinal por duplicación intestinal del íleon terminal. Discutimos la presentación clínica, estudios de imagen, procedimiento quirúrgico y resultados del tratamiento. Revisamos la literatura actual pertinente por ser un hallazgo etiológico de oclusión intestinal poco frecuente. Palabras clave: Duplicación intestinal, oclusión intestinal, abdomen agudo quirúrgico.
Materiales y métodos. Estudio descriptivo, observacional, de corte transversal, y analítico. Fueron incluidos para este estudio pacientes que consultaron en la Unidad de Cirugía Plástica, en un periodo desde noviembre del 2016 a noviembre del 2017. Resultados. El 60 % correspondió a hombres y el promedio de edad de 66 años. El tiempo de evolución desde la aparición de la lesiónfue de 1 a 4 años (40%), en su mayoría derivados de Dermatología (80%). En el 70% de los casos se ubicaban en canto interno exclusivamente; con una medida promedio de 10 mm (75%). El 60 % correspondían a Carcinoma Basocelular Ulcerado Infiltrante. Las reconstrucciones empleadas fueron los colgajos de avance V-Y en un 30%. El25% contaban con un diagnóstico previo. El 15% presentó hematomas en el postoperatorio mediato y la estadía hospitalaria fue de 24 horas. Conclusión. Todas las técnicas de reparación utilizadas demostraron proporcionar resultados funcionales y estéticos satisfactorios, y son altamente recomendables para la reconstrucción de una amplia variedad de defectos del canto interno y áreas circundantes asociadas. Submit the casuistry obtained in the Plastic Surgery Unit of the Hospital of Clinics, in relation to the reconstruction of the internal edge of lesions. Materials and methods. Descriptive, observational, cross-sectional and analytical. Were included in this study patients who consulted in the Unit of Plastic Surgery, in a period from November 2016 to November 2017. Results. The 60 % were men and the average age of 66 years. The time of evolution from the date of the injury was of 1 to 4 years (40%), mostly derived from Dermatology (80%). In the 70% of the cases were located in the internal edge exclusively; with an average measure of 10 mm (75%). The 60 % were ulcerated carcinoma, Basal Cell Carcinoma.
RESUMENObjetivo: Demostrar los efectos funcionales y estéticos de la aplicación de la fracción vascular del estroma derivadas de tejido adiposo autólogo para el tratamiento de secuelas tardías de quemaduras.Materiales y métodos: Estudio analítico, experimental tipo ensayo no controlado con pacientes que presentaban cicatrices por quemaduras, se realizaron mediciones con escalas y microscopía óptica para la evaluación de las características de la cicatriz antes y a los 6 meses del procedimiento.Resultados: No hubo complicaciones locales o sistémicas con respecto al procedimiento y se encontraron mejorías con respecto a las escalas de evaluación y a la microscopía óptica. Por lo que benefician de manera notable la evoluciónde las lesiones, tanto estéticacomo sintomáticamente y en la capacidad funcional así como en la angiogénesis.Conclusión: Laremodelación de las cicatrices utilizando fracción derivadas de tejido adiposo del estroma vascular (SVF) puede plantearse comouna alternativa en el manejo conjunto o aislado de las cicatrices por quemaduras con nula morbilidad para el paciente.Palabras clave: lipoaspiración, cicatrices por quemaduras, fracción derivadas de tejido adiposo del estroma vascular (SVF).
