La microfiltración se define como la percolación de desechos, fluidos orales, microorganismos o iones a través de la interfaz entre el material de restauración y la pared del órgano dentario. Los trazadores más frecuentes que se han utilizado para medir la microfiltración son colorantes, isótopos radiactivos, bacterias y sus productos. El objetivo del artículo es revisar la literatura mundial respecto a las técnicas de medición de microfiltración apical en endodoncia. La presente es una revisión narrativa, que consiste en la lectura y contraste de diferentes fuentes, a través de una búsqueda en las bases de datos digitales: PubMed, SciELO y Google Académico. Treinta y cuatro artículos fueron incluidos en la revisión de literatura, seleccionados de acuerdo a los criterios de inclusión establecidos. Se hallaron las siguientes técnicas: Penetración del tinte filtración de fluidos, disolución del tinte, método de infiltración de bacterias y toxinas, método de presión de aire, método electroquímico, método de radioisótopos, trazadores de solución de metal, método de difusión inversa, caries artificiales, microscopía electrónica de barrido, microscopio electrónico de transmisión, microtomografía computarizada.
Introducción: La microbiota en la terapia endodóntica es considerada una de las causas fundamentales de infecciones microbiológicas. El Enterococcus Faecalis microorganismo esencial tiene la capacidad de sobrevivir en medios adversos dentro de los túbulos dentinarios, sus mecanismos de resistencia, virulencia y patogenicidad son factores relevantes para asociarlo lesiones periapicales persistentes post- tratamiento evidentes en fracasos. Objetivo: El objetivo de esta revisión bibliográfica es conocer los factores de resistencia bacteriana que presenta el Enterococcus Faecalis asociado a los fracasos endodónticos reportados en la literatura a nivel mundial. Metodología: Se realizó una búsqueda en portales como: PubMed, Google Académico, Scielo, Scopus, donde se recopiló información documentada de artículos que dan a conocer los factores de resistencia de Enterococcus Faecalis, estos realizados en diferentes países en los últimos veinte años. Resultados: La mayoría de los artículos reportan que los principales factores de resistencia bacteriana del Enterococcus Faecalis son la producción de distintas sustancias tales como la sustancia de agregación (Cas), producción de proteínas específicas como la proteína de superficie (Esp), Gelatinasa, citolisina, la producción de superóxido extracelular, y como mecanismo de acción la función de la bomba de protones y la producción de beta-lactamasa. Conclusión: Se conoció la complejidad etiológica y los factores de resistencia de la microbiota presente en los conductos radiculares, tales como: producción de proteínas de superficie, presencia de enzimas específicas como: citolisina, hemolisina, muramidasas, gelatinasa, la función de la bomba de protones, la sustancia de agregación y como estructura principal bacteriana , su pared celular.; se destaca la prevalencia del Enterococcus Faecalis principal causal de infección secundaria. Sus habilidades de prevalecer en ambientes extremos, modificar sus capacidades, y su complejos factores de resistencia lleva a esta bacteria a ser un microorganismo potenciado.
La microfiltración coronaria se considera una de las causas de fracaso de los tratamientos de conductos radiculares. La falta de sellado coronario por una inapropiada o la ausencia de la obturación provisional o restauración definitiva permite la penetración desde la cavidad bucal, de microorganismos y sus productos que podrían eventualmente llegar al foramen apical. El objetivo de esta revisión de literatura es comparar la microfiltración de los cementos de obturación provisional en endodoncia, reportados por la literatura mundial. El presente trabajo de investigación es una revisión narrativa, la misma es un tipo de revisión bibliográfica que consiste en la lectura y contraste de diferentes fuentes, a través de una búsqueda en las bases de datos digitales: PubMed, SciELO y Google Académico. Cuarenta artículos fueron incluidos en la revisión de literatura, publicados entre 1978 y 2021, encontrándose conformados por ensayos clínicos aleatorizados y reportes de casos, los artículos fueron seleccionados de acuerdo a los criterios de inclusión establecidos. Se logra diferenciar y comparar la microfiltración a través de diversas técnicas y materiales de restauración provisional. Encontrándose que algunos materiales tienen mejor resistencia a las fuerzas de masticación (IRM); otros tienen una mejor capacidad de sellado (Cavit, Coltosol). Existe un consenso entre varios autores en que el espesor del material debe ser de al menos 3,5 mm para proporcionar un sellado eficaz y que los dientes tratados endodónticamente expuestos a saliva presentan infiltración poco después del 3er día.
