LETTER TO THE EDITORMr. Editor, The frequency of tracheal stenosis after orotracheal intubation varies between 10% and 22%, depending on the consulted series; however, severe stenosis occurs in only 2% of cases 1 . Keloid or hypertrophic scarring, which has an incidence of between 4 and 16% in the Mexican population, is a recognized risk factor for tracheal stenosis that often goes unnoticed 2 . With the idea of highlighting this serious complication, we present a prototype case.A 27-year-old woman with a history of the previous intubation for 9 days secondary to a suicide attempt 21 months earlier was admitted to hospital for stridor and dyspnea with a 34-week pregnancy complicated by symptomatic preeclampsia. During her evaluation, subglottic stenosis with 5-mm airway caliber was corroborated by neck computed tomography (Fig. 1); radiological evaluation included the thoracic surgery department. Antecedentes. Más de 230 millones de cirugías se realizan diario en todo el mundo. Las complicaciones postoperatorias pulmonares (CPP) se asocian con aumento en la mortalidad. La ventilación pulmonar protectora es el estándar de referencia actual para todo paciente que requiera ventilación mecánica. Objetivo. Comparar la presencia de complicaciones postoperatorias inmediatas con estrategias de ventilación protectora contra no protectora, en pacientes bajo anestesia general. Material y métodos. Ensayo clínico abierto, prospectivo, se realizó en un centro único. Se formaron dos grupos: el grupo control no recibió ventilación protectora debido a que el anestesiólogo no respetó el volumen corriente (tidal) (Vt), la presión positiva al final de la espiración (PEEP) y la presión meseta; el grupo estudio recibió ventilación protectora. Se verificó la normalidad de las distribuciones mediante la prueba de Kolmogorov-Smirnov y las diferencias entre los grupos se evaluaron con la prueba t de Student. La muestra se calculó con la fórmula para la estimación de una proporción, prevalencia del 12.9% de CPP ( 22) y un error de 0.05, quedando en 175 pacientes. Resultados. Al comparar los grupos se encontró diferencia estadísticamente significativa (p = 0.03). El grupo que no usó parámetros de protección pulmonar presentó un 43% de CPP en comparación con un 17% del grupo que sí protegió. Conclusión. Los anestesiólogos aún rehúsan respetar los parámetros de ventilación protectora, con uso de volúmenes corrientes altos, sin uso de PEEP, lo que provoca disminución del dióxido de carbono exhalado y mayor CPP.
