INTRODUCCIÓNLa arteritis de Takayasu 1 (AT) es una inflamación granulomatosa de la aorta y sus principales ramas, además afecta principalmente a mujeres. Si bien la mortalidad es baja y la supervivencia durante 10 años es elevada, la evolución puede ser fulminante principalmente en niños, ya que en este grupo alcanza 33%, pero en adultos es extraordinaria. Debido a que presenciamos un caso excepcional en un adulto, en el que la evolución fue fulminante, efectuamos este trabajo.
La leucemia mielomonocítica crónica es un trastorno clonal en el que el cambio afecta el desarrollo normal de los monocitos, afecta a tres de cada 100,000 personas después de los 65 años de edad en los Estados Unidos. Caso clínico: Masculino de 80 años relata inicio de padecimiento 60 días atrás al presentar lesiones cutáneas de tipo nódulo-papulares-eritematosas, algunas confluentes, de bordes bien definidos, algunas de aspecto purpúrico y áreas liqueinificadas por rascado, diseminadas a extremidades, respetando cara, palmas de manos y plantas de pies; con hepatomegalia de 2 cm y esplenomegalia de 3 cm por debajo de borde costal; la fórmula blanca mostró: monocitos 27%, plaquetas 40,000/mcL. Al examen de frotis de sangre: metamielocitos 2%; mielocitos 1%; vacuolización citoplasmática de monocitos, basofilia difusa y normoblastos 1%. El examen inmunofenotípico mostró: aumento de SS (granularidad) en la población de monocitos, disminución en la expresión de CD4 y HLA-DR, sobreexpresión de CD123 y CD16, además de expresión aberrante de CD56 en 32.8% del total de leucocitos; la biopsia de piel mostró: piel con infiltración por células neoplásicas malignas dispuestas en nidos, sábanas y/o cordones, con marcador de inmunohistoquímica mieloperoxidasa positivo. Conclusión: Se estableció diagnóstico de leucemia mielomonocítica crónica con infiltración cutánea.
La endometriosis es una enfermedad frecuente, que se caracteriza por tejido endometrial fuera del útero. La endometriosis apendicular corresponde a 3% de los casos con afección gastrointestinal y menos de 1% de todos los casos de endometriosis. Caso clínico: Mujer de 27 años de edad que fue ingresada a Urgencias por dolor abdominal en el cuadrante inferior derecho de 12 horas de evolución, acompañado de náusea y vómito en una ocasión y tres evacuaciones diarreicas. En la exploración física se encuentra con FC 90 l/min; TA 120/80 mmHg; FR 18 min; afebril, sin ataque al estado general; abdomen con peristalsis presente, dolor intenso en el cuadrante inferior derecho, signos de McBurney, Rovsing, Von Blumberg y psoas positivos. Los estudios de laboratorio reportaron leucocitosis de 11,800 con 87% de segmentados; el ultrasonido reportó imagen sugestiva de proceso apendicular agudo, apéndice de 6.6 mm en su diámetro transverso y presencia de ganglios periapendiculares. Se le efectuó apendicectomía por laparoscopia. Patología reportó periapendicitis aguda local, secundaria a endometriosis apendicular con focos múltiples a nivel de la capa muscular y subserosa. Conclusiones: La endometriosis afecta al apéndice cecal en 3% de los casos, por lo que debe estar presente en el diagnóstico diferencial de los casos de abdomen agudo.
