Fecha de recepción: 23 de diciembre de 2017 -Fecha de aceptación: 11 de enero de 2018Resumen: La granulomatosis de Wegener es una vasculitis sistémica primaria que afecta el tracto respiratorio superior e inferior, los glomerulos renales y desarrolla vasculitis de vasos de mediano y pequeño calibre. Se asocia a anticuerpos anticitoplasma de neutrofilos (ANCA), por lo que hoy en dia se agrupa dentro de las vasculitis ANCA positivas. La característica anatomopatológica más relevante es la presencia de granulomas necrosantes. La afección pulmonar abarca desde nódulos pulmonares asintomáticos a infiltrados pulmonares y hemorragias alveolares fulminantes. Es una vasculitis poco frecuente en Ecuador, su pronóstico es malo debido a insuficiencia renal o respiratoria, aunque las cifras han cambiado gracias a los nuevos tratamientos con glucocorticoides y ciclofosfamida. Se presenta un caso con afectación pulmonar severa que tras una adecuada anamnesis y las pruebas correspondientes se llegó al diagnóstico de la enfermedad descrita previamente.Palabras Clave-Granulomatosis de Wegener, hemorragia alveolar, vasculitis.Abstract: Wegener's granulomatosis is a primary systemic vasculitis that affects the upper and lower respiratory tract, the renal glomeruli and develops vasculitis of medium and small caliber vessels. It is associated with antineutrophil cytoplasmic antibodies (ANCA), so that today it is grouped within the ANCA positive vasculitis. The most relevant anatomopathological characteristic is the presence of necrotizing granulomas. Lung involvement ranges from asymptomatic pulmonary nodules to pulmonary infiltrates and fulminating alveolar hemorrhages. It is a rare vasculitis in Ecuador, its prognosis is bad due to renal or respiratory failure, although the figures have changed thanks to new treatments with glucocorticoids and cyclophosphamide. We present a case with severe pulmonary involvement that after an adequate anamnesis and the corresponding tests, the diagnosis of the previously described disease was reached.Keywords-Wegener's granulomatosis, alveolar hemorrhage, vasculitis. INTRODUCCIÓN L a granulomatosis de Wegener es una vasculitis necrotizante de medianos y pequeños vasos, con inflamación granulomatosa del tracto respiratorio superior e inferior como los riñones.En 1931, Heinz Klinger describió por primera vez el caso. En 1954, Jacob Godman et al. ampliaron estos conceptos y depuraron los aspectos patológicos de la enfermedad. En los años 1980 se observó que los anticuerpos citoplásmicos antineutrófilos (ANCA), tenían especificidad del 95 % para el diagnóstico; también se encontró que los anticuerpos dirigidos contra la proteinasa 3 (PR3) se correlacionaban con la existencia de ANCA y de este tipo de granulomatosis.Los síntomas de esta patología se relacionan con la afectación del tracto respiratorio superior (nariz, senos paranasales y oídos), del tracto respiratorio inferior y los riñones, completando todo ello la tríada clásica de esta enfermedad.La afección pulmonar abarca desde la presencia de nód...
