con enfermedad renal crónica (ERC) que requieren tratamiento dialítico de urgencia. Método: Estudio de cohorte histórica, observacional, longitudinal, retrolectivo y analítico. Se analizaron hombres y mujeres derechohabientes de la institución con diagnóstico de ERC y necesidad de tratamiento dialítico y para colocación por primera vez de catéter de Tenckhoff en el periodo comprendido de enero de 2016 a diciembre de 2017. Se incluyeron todos los casos durante el periodo de estudio. Resultados: A 135 pacientes de 245 con diagnóstico de ERC se les colocó un catéter de Tenckhoff en las primeras 24 horas. El tiempo medio para la colocación del catéter fue de 6 horas (rango intercuartílico [RIQ]: 8 horas) para el grupo A y de 26.5 horas (RIQ: 15 horas) para el grupo B. La mediana de horas de estancia intrahospitalaria para el grupo A fue de 9 horas (RIQ: 11 horas), mientras que para el grupo B fue de 28.5 horas (RIQ: 14 horas). La mediana de vida del catéter para el grupo A fue de 12 meses (RIQ: 7 meses) y para el grupo B fue de 8 meses (RIQ: 10 meses). Conclusiones: La colocación de un catéter de Tenckhoff en las primeras 24 horas del ingreso a la sala de urgencias se asoció con el tiempo de vida media del catéter, de manera que, a menor tiempo de estancia intrahospitalaria y menor tiempo para su colocación, mayor será el tiempo de funcionalidad libre de complicaciones.
Introducción: A inicios de la pandemia por el nuevo coronavirus 2 del síndrome respiratorio agudo grave (SARS-CoV-2) se estableció que el tratamiento con cloroquina podía ser efectivo para contener la replicación viral in vitro, pero aún existe controversia sobre su efectividad en humanos para disminuir la mortalidad por enfermedad por coronavirus 2019 (COVID-19) en pacientes graves. Objetivo: Evaluar la mortalidad asociada al uso de cloroquina en pacientes con infección grave por SARS-CoV-2. Métodos: Estudio de cohorte retrospectiva, en un hospital de 2.º nivel. Se revisaron 171 expedientes de pacientes hospitalizados por COVID-19 grave, se analizó el riesgo de mortalidad por uso de cloroquina. Se analizaron datos sociodemográficos, patologías previas, días desde el inicio de los síntomas hasta la atención médica, días de estancia hospitalaria, parámetros respiratorios; gasometría, citometría hemática, deshidrogenasa láctica, proteína C reactiva (PCR), tratamiento antiviral, tromboprofilaxis, uso de esteroides y uso de antibióticos. Se utilizó estadística descriptiva, inferencial, modelos univariantes y multivariantes. Resultados: El uso de cloroquina, la edad avanzada, la obesidad y la PCR baja se asociaron con una mayor probabilidad de muerte en el análisis univariante. La edad (hazard ratio [HR]: 0.976; intervalo de confianza del 95% [IC 95%]: 0.959-0.993), no padecer hipertensión (HR: 0.788; IC 95%: 0.620-1.00) y el uso de cloroquina (HR: 2.07; IC 95%: 1.2-3.4), fueron factores de riesgo significativos asociados con la muerte en los casos con COVID-19 grave. Conclusiones: En este estudio encontramos evidencia para recomendar evitar el uso de cloroquina en pacientes adultos con COVID-19 grave debido al aumento en el riesgo de mortalidad hasta dos veces más en comparación con los que no se les administró.
