Introducción: los materiales cerámicos infiltrados con polímero CAD/CAM necesitan tratamientos micromecánicos y químicos que modifiquen su superficie y favorezcan la adhesión a dentina de restauraciones indirectas. Objetivo: evaluar in vitro la fuerza microtensil que causa la combinación del tratamiento superficial de microarenado (MA), más ácido fluorhídrico (HF 10%), al aplicar un adhesivo universal (AU) sobre especímenes de Lava™ Ultimate (LVU). Material y métodos: se utilizó una nanocerámica de resina LVU (3M ESPE, St. Paul, MN, USA). Las muestras se dividieron en cuatro grupos. Grupo 1: control (sin tratamiento). Grupo 2: MA con Al2O3 de 50 μm (Zeta Sand Zhermarck Dental SpA, Italia) + AU (All-Bond Universal® Bisco Inc. Schaumburg, IL). Grupo 3: grabado con AF al 10% Angelus® (Solucoes Odontologicas, Londrina, Brazil) + AU. Grupo 4: MA +AF + AU. Los especímenes se cementaron con resina dual Duo-Link™ (Bisco Inc. Schaumburg, IL) en dentina media de terceros molares extraídos. Después de 24 horas se realizó la prueba de microtracción. El modo de fallo de las muestras se registró bajo estereomicroscopio a 20X. La rugosidad y aspecto superficial se evaluaron con microscopio electrónico de barrido (MEB) y microscopio de fuerza atómica (MFA). Resultados: el tratamiento de superficie de LVU con MA + AF + AU mostró diferencias significativas en la resistencia microtraccional (p < 0.011). El modo de fallo predominante en los grupos fue adhesivo. Se identificó una correlación significativa, al observar que a mayor rugosidad de la superficie tratada, había mayor resistencia microtraccional (r = 0.88) Conclusiones: la resina nanocerámica LVU grabada con MA y con AF crea una rugosidad superficial que combinada con un AU incrementa la resistencia de fuerza adhesiva en dentina.
Introducción: los instrumentos endodóncicos usados retienen debris dentinario, lo que representa un riesgo biológico que afecta su esterilización. Objetivo: evaluar con la técnica de espectroscopia de plasmas inducida por láser (LIBS) la limpieza de instrumentos endodóncicos de NiTi tratados con láser Nd:Yag. Material y métodos: se evaluaron instrumentos ProTaper Next X2 (Dentsply Maillefer®, Ballaigues, Switzerland) y WaveOne Gold Primary (Dentsply Maillefer®, Ballaigues, Switzerland) bajo microscopio estereoscópi- co (20X) en dos etapas: 1) instrumentos usados, 2) instrumentos limpiados con ultrasonido o con láser Nd:Yag. En ambas etapas se registró la presencia y extensión de debris dentinario teñido con so- lución Van Gieson. Posteriormente, se analizó con técnica LIBS los elementos contenidos en el debris dentinario en instrumentos NiTi usados y tratados con diferentes métodos de limpieza. Resultados: la técnica láser Nd:Yag presentó mayor capacidad de limpieza (p < 0.0001). El análisis LIBS no mostró espectros de elementos conte- nidos en el debris biológico en los instrumentos tratados con ambos métodos. Conclusiones: el láser Nd:Yag es un método eficaz para la limpieza de instrumentos NiTi que elimina de su superficie los elementos contenidos en el debris dentinario.
Introducción: la remoción completa de materiales de obturación y dentina contaminada del conducto radicular durante el retratamiento endodóntico puede influir en la aparición de agudización endodóntica. Objetivo: establecer la frecuencia de agudización endodóntica posterior al retratamiento endodóntico de dientes anterosuperiores con diferente longitud de obturación apical. Material y métodos: se eligieron 33 casos con fracaso endodóntico para este estudio. Según la longitud apical de la obturación inicial, los sujetos se dividieron en dos grupos: el grupo A (n = 23) fueron casos obturados entre 0.0 y 2.0 mm al ápice radiográfico (AR) y el grupo B (n = 10) casos obturados entre 2.1 y 6 mm cortos al AR. Las obturaciones con gutapercha se eliminaron con instrumentos ProTaper retreatment D1 a D3 (Dentsply-Sirona, Ballaigues, Switzerland) más cloroformo. Posteriormente los conductos radiculares se instrumentaron con limas K-flexofile. Los casos reinstrumentados fueron obturados con gutapercha (Higyenic, Coltene Whaledent) y Sealapex (Kerr Sybronendo, WA) durante la misma cita. Los pacientes tomaron 400 mg de ibuprofeno cada ocho horas durante 72 horas y registraron el dolor postoperatorio a las ocho, 24, 48 y 72 horas. Se definió como agudización la aparición de dolor severo y/o inflamación (dolor en escala Heft-Parker a partir de 114 mm). Se determinaron porcentajes de casos positivos a la palpación y percusión a las 72 horas en ambos grupos. Resultados: dos pacientes presentaron agudización (6%), ambos en el grupo A. El dolor postoperatorio fue de 21.2% en los pacientes del grupo A a las ocho horas y 9.1% a las 48 horas. No se encontraron diferencias estadísticamente significativas en la presencia de agudización entre grupos (p > 0.05). Conclusiones: la agudización de casos de retratamiento endodóntico es de 6%. La longitud de obturación de casos indicados para retratamiento endodóntico no influye en la frecuencia de agudización postoperatoria.
The objective of this study was to evaluate in vitro the effect of pH on the mechanical properties of three different acetates brands used to manufacture dental aligners. MATERIALS AND METHODS: 27 specimens of thermoformed acetates of the brands Zendura A., Dentsply Sirona and Valdi were incubated in artificial saliva at 37°C in a Shel-Lab 1510E incubator and immersed twice a day for 5 minutes in beverages with different pH levels to measure its elastic limit on days 0, 7 and 14. STATISTICAL PROCESS CONTROL: Data analysis was performed using the KolmorogovSmirnov and Levene tests, and later with analysis of variance (ANOVA), multiple comparisons post -hoc with the Sheffe test and the Friedman statistical test for paired samples; all with a significance level of 0.05 using the IBM SPSS Statistics 26 statistical package. RESULTS: The elastic limit measurements on days 7 and 14 indicate significant differences between the different pHs and are not consistent between the three brands of acetates. The Zendura A brand at neutral pH increased and then recovered to 99%, at acidic pH it decreased to 85% then 64%, and at alkaline pH it increased more than half, then returned to 105% of baseline. The Dentsply Sirona brand in neutral pH had a decrease and then recovered to 96%, in acidic pH it increased progressively to 113% and 118%, and in alkaline pH it decreased to 69% to later recover to 84%. The Valdi brand in all groups increased and at the end was recovered, in neutral pH to 92%, in acidic pH to 87%, and in alkaline pH to 80% with respect to the initial value. CONCLUSIONS: The mechanical properties of the aligners vary and may be affected depending on the pH to which they are subjected, having variability in their elastic limit. The specimens subjected to acidic and alkaline drinks had a greater decrease in their elastic limit when compared to those subjected to neutral pH. In general, it was possible to observe that the specimens increased their elastic limit on day 7 and then approached their initial value on day 14.
