Background: Most patients infected with Helicobacter pylori show no clinical symptoms. Nonetheless, approximately 10% to 20% of these patients will develop peptic ulcers and 1% will develop gastric cancer. The International Agency for Cancer Research has classified Helicobacter pylori as a Group 1 carcinogen. Materials and Methods: Drinking water samples from 20 cantons situated in areas of low and high incidence of gastric cancer in Costa Rica were analyzed. Water samples were concentrated and the nitrocellulose filter was cultivated for later molecular identification of Helicobacter pylori using the glmM marker. The tumor necrosis factor-alpha inducing protein (Tipalpha TNF-α) was used as a pathogenicity marker of the strains found followed by the sequencing of two products. Information on water management by aqueduct operators was collected in order to establish statistical relationships. Results: A total of 112 water samples were analyzed. Successful cultivation and molecular identification using the glmM marker was achieved in 39% of samples from areas of high incidence and in 7.5% of samples from areas of low incidence. Two glmM gene PCR products were sequenced and confirmed as true positives. Conclusions: The highest prevalence rate of Helicobacter pylori was found in water from areas with a high incidence of gastric cancer. Additionally, significant statistical correlations were established among environmental conditions, drinking water management and incidence of gastric cancer.
Un hato lechero eficiente debe producir una cría viva y sana por vaca, por año, y esto se debe de conseguir con un Intervalo Parto Concepción (IPC) no superior a 100 días. Objetivo: describir aspectos epidemiológicos del IPC en vacas de
fincas de lechería especializada de Costa Rica; en el período 2000-2013. Materiales y métodos: estudio prospectivo histórico con 434 708 registros del programa VAMPP Bovino 3.0. Se estimó el IPC para variables extrínsecas e intrínsecas de las vacas.
Se analizaron los datos mediante estadística descriptiva (promedio, mediana, desviación estándar, intervalo de confianza al 95 %); además, se realizó comparaciones de promedios y de porcentajes por medio de la prueba de T de Student y Chi-cuadrado,
respectivamente. También, se estimó el riesgo de IPC mayor a 100 días, calculando la razón de tasas (RR) mediante Regresión de Poisson. Resultados: más del 50 % de las vacas presentaron un IPC superior a 100 días; con un promedio general de 136,6
días (± 84,2). El IPC fue más alto en animales de la primera lactancia (142,0 ± 88,4 días) (P<0,001) y mayor en vacas Holstein (148,7±85,9 días) que en Pardo Suizo y Jersey (P < 0.0001). El promedio del IPC fue más alto en vacas con parto
distócico o cesárea (P < 0,001); igualmente, las de alta producción de leche (5900-10100 kg/305d) fueron las de mayor IPC (141,5 d ± 80,8 d). Las vacas pertenecientes a regiones de bosque seco presentaron casi 10 días más de IPC que las otras
zonas (P<0,01). Esas mismas condiciones fueron las que mostraron el mayor riesgo (razones de tasas) de IPC mayores a 100 días. Conclusión: el IPC se ve influenciado por variables extrínsecas e intrínsecas. Por lo que, el enfoque de manejo del hato
debe de estar relacionado con las variables modificables, en especial la observación de celos y la eficiencia en la inseminación artificial.
