De organisatie in een huisartsenpraktijk is net zo simpel als die in een gewone huishouding met twee kindertjes. Onze oma's hadden wel tien kinderen en kregen die zonder verbeterprojecten allemaal groot. Hoeveel valt er dan eigenlijk te managen in een twee-, drie-of viermanspraktijk? Merkwaardig dat een NHG-congres dat op de presentatie van de Toekomstvisie volgt, toch geheel gewijd is aan de praktijkorganisatie. Drastische veranderingen in de wereld om ons heen maken het blijkbaar noodzakelijk dat we onze aandacht vooral richten op het vernieuwen van de praktijkorganisatie. Het plezier in het werk dat velen verloren zouden hebben of dreigen te verliezen, zou weer terugkomen als we de handen maar uit de mouwen steken en 'innoverende veranderingen in de praktijkorganisatie' doorvoeren. Vorige maand betoogden Verstappen en Hobma dat en begin dit jaar hield ook Achterbergh al een pleidooi voor een andere organisatie.1,2 De laatste jaren lijkt de aandacht voor allerlei aspecten van praktijkvoering fors toe te nemen. H&W krijgt geregeld manuscripten aangeboden waarin verslag wordt gedaan van een project over praktijkvoering of bepaalde onderdelen daarvan. Van der Werf en De Haan geven in dit nummer een overzicht van de thema's op het gebied van praktijkvoering die sinds 1957 in Huisarts en Wetenschap opgedoken zijn.3 Praktijkvoering blijkt een veelomvattend begrip: de praktijkorganisatie, de financiering, de administratie, categorale spreekuren, preventieve activiteiten, de taakstelling van de praktijkassistente, maar ook kwaliteitsverbeterprojecten kunnen onder de noemer praktijkvoering geschaard worden. Eigenlijk vallen alleen medisch-inhoudelijke aspecten en de arts-patie¨ntcommunicatie erbuiten. Waarom sommige onderwerpen opeens op het toneel verschijnen, in de mist oplossen en tien jaar later opeens weer opduiken, blijft onduidelijk. Zij besluiten hun artikel met de uitspraak dat een wetenschappelijke discussie op het gebied van de praktijkvoering noodzakelijk is. Als praktijkvoering tussen scalpel en automatisering, tussen zorg voor specifieke groepen en werkgeversschap zwalkt, dan is het de vraag waarover die wetenschappelijke discussie zou moeten gaan: over het raakvlak met de medisch-inhoudelijke kant van de zorg of over het pure regelwerk? We willen in dit commentaar wel een voorzetje geven.Veel projecten op het gebied van de praktijkvoering gaan uit van een vaak niet expliciet gemaakte gedachte dat verbetering van praktijkvoering tot verbetering van de kwaliteit van de zorg leidt. Maar wat verstaan die verbeteraars onder kwaliteit van zorg? Is er pas sprake van verbetering van de kwaliteit als de gezondheid van de patie¨nt er op vooruitgaat? Is verbetering van zorg het efficie¨nt verlenen van zorg en het bestaan van korte wachtlijsten? Of is het ook al goed als patie¨nten meer tevreden zijn? Als we er voor het gemak van uitgaan dat deze drie omschrijvingen van kwaliteitsverbetering goede uitkomstmaten zijn, dan is de vraag in hoeverre kwaliteitsverbeterprojecten de beoogde doelen halen. Simpele kop, simpel antwoord: een goed tijdschrift maken. Het is onze zorg om elke vier weken een leesbaar tijdschrift met wetenschappelijke informatie voor huisartsen te maken. De inhoud moet een mix zijn van onderzoek, nascholing, klinische lessen en discussie, gelardeerd met nieuws, Cochrane-reviews en besprekingen. Dat tijdschrift moet vooral ook leesbaar zijn. Wat kost er tijd?In 2001 beoordeelden we 136 manuscripten, waarvan we er 43 zelf gevraagd hadden. We wezen 45 artikelen af, vrijwel altijd om niet op te lossen problemen in de onderzoeksmethoden of vanwege een gebrek aan relevantie. Twee manuscripten accepteerden we direct; alle andere keren vroegen we de auteurs om allerhande wijzigingen. Dat beoordelen en corresponderen kost de redactieleden elk ongeveer een halve dag in de week en het redactiesecretariaat een aanzienlijk deel van de werkweek. Met enige regelmaat leggen auteurs eerste concepten ter beoordeling aan de voorzitter van de redactiecommissie voor. Soms leidt dat tot indiening van een 'echt manuscript' soms tot een directe afwijzing en soms tot een advies het elders eens te proberen. Cijfers houden we hierover niet bij. We publiceerden in 2001 83 artikelen, 8 standaarden, 2 Landelijke transmurale afspraken, enkele aanvullingen op de standaarden en veel 'fait divers'. In 2001 liep de succesvolle serie 'Zwangerschap en Kraambed' af en startten we een nieuwe onder de titel 'Terug naar Woudschoten'. In samenwerking met stafleden van de afdelingen studentenon-derwijs van de acht afdelingen huisartsgeneeskunde publiceerden we artikelen over diagnostiek van alledaagse aandoeningen. Eind 2001 startten we met een maandelijkse pagina met cijfers uit LINH. Na een gedegen voorbereiding door de redactiecommissie, redactiesecretariaat en directie nam in maart 2001 NHG-bestuur het besluit tot een restyling van het tijdschrift. Nummer 10, september, kwam uit in een geheel nieuw vormgeven jas: andere broodletter, full colorkleuren-druk, plaatjes, meer nieuws, 'herintredende' kennistoets en tal van kleine verbeteringen. Niet alleen de buitenkant en letters zijn veranderd, H&W wordt ook langzaam echt een beetje anders. Wat is er niet gelukt?De belofte om het cumulatieve register van H&W op internet toegankelijk te maken is door technische problemen niet waargemaakt. Ook is de site van H&W nog steeds betrekkelijk rudimentair: er zijn nog steeds alleen abstracts te vinden. In 2000 maakten we plannen voor een serie over cognitieve en psychiatrische problemen bij ouderen. Daar publiceren we in 2002 wel enige artikelen uit, maar erg hard loopt het niet.De in 2000 gesignaleerde trend van een toename aan brieven is helaas weer omgebogen, huisartsen hebben het blijkbaar te druk met andere zaken. Plannen voor 2002Vanaf januari 2002 maakt In de praktijk vast onderdeel uit van H&W. Dit katern probeert -onder een aparte redactie -vooral aandacht te geven aan implementatie van het kwaliteitsbeleid.Dit jaar moet de website ook echt een 'beter gezicht' krijgen, zodat er een toegankelijk elektronisch archief van H&W-artikele...
