Several-year long deportation in childhood and PTSD elevated overall cardiovascular risk in the group of former deportees compared with the group of persons without history of such a traumatic experience.
Objectives. To evaluate the spectrum of neurological symptoms in patients with COVID-19 during the first 14 days of hospitalisation and its association with in-hospital mortality. Material and methods.We included 200 patients with RT-PCR-confirmed COVID-19 admitted to University Hospital in Krakow, Poland. In 164 patients, a detailed questionnaire concerning neurological symptoms and signs was performed prospectively within 14 days of hospitalisation. In the remaining 36 patients, such questionnaires were completed retrospectively based on daily observations in the Department of Neurology.Results. During hospitalisation, 169 patients (84.5%) experienced neurological symptoms; the most common were: fatigue (62.5%), decreased mood (45.5%), myalgia (43.5%), and muscle weakness (42.5%). Patients who died during hospitalisation compared to the remainder were older (79 [70.5-88.5] vs. 63.5 [51-77] years, p = 0.001), and more often had decreased level of consciousness (50.0% vs. 9.3%, p < 0.001), delirium (33.3% vs. 4.4%, p < 0.001), arterial hypotension (50.0% vs. 19.6%, p = 0.005) or stroke during (18.8% vs. 3.3%, p = 0.026) or before hospitalisation (50.0% vs. 7.1, p < 0.001), whereas those who survived more often suffered from headache (42.1% vs. 0%, p = 0.012) or decreased mood (51.7% vs. 0%, p = 0.003). www.journals.viamedica.pl/neurologia_neurochirurgia_polska Marcin Wnuk et al., Neurological symptoms in COVID-19 Conclusions. Most hospitalised patients with COVID-19 experience neurological symptoms. Background In contrast with the setting of acute myocardial infarction, there are limited data regarding the impact of diabetes mellitus on clinical outcomes in contemporary cohorts of patients with chronic coronary syndromes. We aimed to investigate the prevalence and prognostic impact of diabetes according to geographical regions and ethnicity. Methods and results CLARIFY is an observational registry of patients with chronic coronary syndromes, enrolled across 45 countries in Europe, Asia, America, Middle East, Australia, and Africa in 2009–2010, and followed up yearly for 5 years. Chronic coronary syndromes were defined by ≥1 of the following criteria: prior myocardial infarction, evidence of coronary stenosis >50%, proven symptomatic myocardial ischaemia, or prior revascularization procedure. Among 32 694 patients, 9502 (29%) had diabetes, with a regional prevalence ranging from below 20% in Northern Europe to ∼60% in the Gulf countries. In a multivariable-adjusted Cox proportional hazards model, diabetes was associated with increased risks for the primary outcome (cardiovascular death, myocardial infarction, or stroke) with an adjusted hazard ratio of 1.28 (95% confidence interval 1.18, 1.39) and for all secondary outcomes (all-cause and cardiovascular mortality, myocardial infarction, stroke, heart failure, and coronary revascularization). Differences on outcomes according to geography and ethnicity were modest. Conclusion In patients with chronic coronary syndromes, diabetes is independently associated with mortality and cardiovascular events, including heart failure, which is not accounted by demographics, prior medical history, left ventricular ejection fraction, or use of secondary prevention medication. Cel pracyNadciśnienie tętnicze jest chorobą, w przebiegu której mogą pojawić się powikłania narządowe. Niniejsze badanie porównuje różne cechy osobowości u pacjentów z nadciśnieniem tętniczym, u których stwierdzono lub też nie przerost lewej komory serca i sztywność dużych naczyń tętniczych.MetodaGrupa badana składała się z 93 osób (47 mężczyzn i 46 kobiet) z rozpoznanym nadciśnieniem tętniczym pierwotnym. U każdego z pacjentów wykonano pomiar wskaźnika masy lewej komory serca (LVMI) oraz prędkości fali tętna (PWV). Uzyskane wartości liczbowe posłużyły jako wskaźniki powikłań narządowych nadciśnienia tętniczego w obrębie serca oraz dużych naczyń tętniczych. Do oceny cech osobowości uczestników badania wykorzystano trzy narzędzia psychometryczne: NEO-FFI, DS14 oraz EAS.WynikiPacjenci, u których stwierdzono podwyższone wartości wskaźnika PWV wykazywali istotnie niższe wyniki w porównaniu z osobami, u których wykryto prawidłowe wartości tego wskaźnika w następujących skalach: NEO-FFI Neurotyczność (18 vs. 27,5; p=0,018), DS14 Negatywna emocjonalność (11,5 vs. 17; p=0,035) oraz EAS Strach (10 vs. 13; p=0,004). Uczestnicy badania, u których stwierdzono przerost lewej komory serca (podwyższone wartości wskaźnika LVMI) uzyskiwali niższe wyniki w skali otwartość na doświadczenie inwentarza NEO-FFI niż pacjenci, u których wskaźnik LVMI miał wartość prawidłową (23 vs. 26; p=0,027).WnioskiWyniki niniejszego badania sugerują, że istnieją istotne różnice w zakresie cech osobowości u pacjentów, którzy w przebiegu nadciśnienia tętniczego rozwijają lub też nie powikłania narządowe w obrębie serca i dużych naczyń tętniczych.
BackgroundThe hypothesis that traumatic experiences in early childhood impact personality formation and psychopathology is well known in psychology and psychiatry, but this is difficult to verify statistically in methodological terms. The aim of this study, conducted with politically persecuted Poles, was to establish the influence of the time when trauma is experienced on the development of psychopathological symptoms.MethodsThe subjects were divided into two groups: those who had experienced trauma before age five (group 1) and those who experienced trauma at an older age (group 2). Subjects in both groups suffered from chronic untreated post-traumatic stress disorder. In order to test the research hypothesis, the Minnesota Multiphasic Personality Inventory-2 profiles of both groups were compared using Student’s t-test, and the Mann–Whitney U-test.ResultsStatistically significant between-group differences were found for the F validity scale and the following clinical scales: Hypochondriasis, Depression, Psychopathic deviate, Psychasthenia, Schizophrenia, and Social introversion. All the significantly different scores were higher in the group traumatized in early childhood. People exposed to trauma under age five had profiles similar to those traumatized after age five, but they experienced their symptoms more intensely.ConclusionsOf clinical significance, higher scores on the psychasthenia, schizophrenia, and social introversion scales, especially on the psychopathic deviate scale, indicated pathology only in the early childhood trauma group. Taken together, these symptoms lead to withdrawal and hindrance of social functioning. This outcome confirms the hypothesis of the influence of various early childhood factors (such as trauma) on personality formation and personality traits in adulthood.
Cel pracyNadciśnienie tętnicze może być powikłane uszkodzeniami narządowymi (oznaczanymi w języku angielskim skrótem TOD – target organ damage). Do chwili obecnej udało się zidentyfikować wiele różnych czynników powiązanych z rozwojem TOD. Jednak, zgodnie z naszą wiedzą, nie próbowano wyodrębnić zmiennych psychologicznych, które różnicują pacjentów z nadciśnieniem oraz uszkodzeniami narządowymi od osób z nadciśnieniem, ale bez TOD. Niniejsze badanie porównuje poziom poczucia koherencji u osób chorujących na nadciśnienie tętnicze powikłane sztywnością dużych naczyń tętniczych lub przerostem lewej komory serca oraz u pacjentów chorujących na nadciśnienie tętnicze niepowikłane uszkodzeniami narządowymi.MetodaGrupa badana składała się z 93 osób. Każdej z osób badanych zaproponowano udział w następujących procedurach: w ocenie klinicznej, w badaniu echokardiograficznym (aby ocenić uszkodzenia wywołane nadciśnieniem tętniczym w obrębie mięśnia sercowego), w pomiarze prędkości fali tętna (aby ocenić uszkodzenia w obrębie naczyń tętniczych) oraz w badaniu psychologicznym (mierzącym poziom poczucia zrozumiałości, zaradności, sensowności oraz koherencji).WynikiPacjenci z uszkodzeniami w obrębie naczyń tętniczych (z wysoką prękością fali tętna) mieli wyższy poziom poczucia zrozumiałości oraz poczucia koherencji niż inne osoby z nadciśnieniem tętniczym. Z kolei uczestnicy badania, u których stwierdzono przerost lewej komory serca nie różnili się pod względem poziomu poczucia koherencji (i wszystkich jego składowych) od pacjentów z prawidłowymi wymiarami lewej komory serca.WnioskiWyniki niniejszego badania sugerują, że poczucie koherencji może nie być wystarczająco silnie związane z dobrym zdrowiem somatycznym. Stanowią także przyczynek do dyskusji nad wartością stosowania SOC-29 jako izolowanego narzędzia. Z pewnością warto byłoby uwzględnić pomiar poziomu poczucia koherencji w kolejnych badaniach poświęconych mechanizmom rozwoju uszkodzeń narządowych w przebiegu nadciśnienia tętniczego.
