Die Teledermatologie hat in den deutschsprachigen Ländern wie auch weltweit einen zunehmenden Stellenwert und bietet aufgrund des hohen Innovationsgrades eine Vorreiter- und Vorbildfunktion für weitere telemedizinische Anwendungen anderer Fachrichtungen. Eine qualitätsgesicherte teledermatologische Behandlung ist in den deutschsprachigen Ländern praktikabel und kann zu einem relevanten Mehrnutzen in der Versorgung führen. Ihr Einsatz ist immer dann in Erwägung zu ziehen, wenn relevante Zusatznutzen für die Patienten ohne relevante Nachteile für sie und für die Versorgenden zu erwarten sind. Für die teledermatologische Behandlung wurden mit dem vorliegenden Konsensuspapier praxisrelevante Maßgaben festgelegt. Etwaige situationsabhängige Limitationen in der Versorgung sind stets zu beachten.
Recent experimental and computational studies have shown that transmurally heterogeneous material properties through the arterial wall are critical to understanding the heterogeneous expressions of constituent degrading molecules. Given that expression of such molecules is thought to be intimately linked to local magnitudes of stress, modelling the transmural stress distribution is critical to understanding arterial adaption during disease. The aim of this study was to develop an arterial growth and remodelling framework that can incorporate both transmurally heterogeneous constituent distributions and residual stresses, into a 3-D finite element model. As an illustrative example, we model the development of a fusiform aneurysm and investigate the effects of elastinous and collagenous heterogeneities on the stress distribution during evolution. It is observed that the adaptive processes of growth and remodelling exhibit transmural variations. For physiological heterogeneous constituent distributions, a stress peak appears in the media towards the intima, and a stress plateau occurs towards the adventitia. These features can be primarily attributed to the underlying heterogeneity of elastinous constituents. During arterial adaption, the collagen strain is regulated to remain in its homoeostatic level; consequently, the partial stress of collagen has less influence on the total stress than the elastin. However, following significant elastin degradation, collagen plays the dominant role for the transmural stress profile and a marked stress peak occurs towards the adventitia. We conclude that to improve our understanding of the arterial adaption and the aetiology of arterial disease, there is a need to: quantify transmural constituent distributions during histopathological examinations, understand and model the role of the evolving transmural stress distribution.
Auf dem Weg zur digitalen Brücke existieren bereits erste Lösungen, die den Lebenszyklus einer Brücke abbilden können. Für die Planung, den Bau und den Unterhalt stehen unterschiedlichste Werkzeuge, z. B. BIM, DIN 1076, SIB‐Bauwerke, Monitoring etc. zur Verfügung, die jeweils mit unterschiedlichen Datenformaten arbeiten. Für ein intelligentes Erhaltungsmanagement müssen aber alle Daten mit den verschiedenen Datenformaten zusammengeführt, abgelegt und so verwaltet werden können, dass über den gesamten Lebenszyklus einer Brücke die Abbildung eines ganzheitlichen digitalen Zwillings eines Bauwerks möglich ist. Die Autoren haben dafür mit BBox den Prototyp einer Verwaltungsschale (VWS) auf Grundlage von Industrie 4.0 entwickelt. Damit wird das physikalisch‐ingenieurtechnische Modell zur Zustandsbewertung der Brücke in den Mittelpunkt gestellt und der gesamte Lebenszyklus einer Brücke kann unabhängig von Datenformaten digital erfasst werden. Da der Aufbau der VWS durch die Granularität optimal strukturiert ist, bietet die Ablage und Einspeisung von Messdaten sowohl die Grundlage eines Live‐Monitorings als auch den Grundstein für maschinelles Lernen (ML). Der Datenzugriff via S3‐Schnittstelle erleichtert die Entwicklung von eigenen Prognosemodellen mit Informationsmustern (SHIP – Structural Health Information Pattern). Am Beispiel der Heinrichsbrücke Bamberg wird die praktische intelligente Umsetzung des Bauwerksmonitorings inkl. VWS mit Informationsmustern und ML gezeigt.
