Zusammenfassung. Hintergrund: Die Aufrechterhaltung sowie Steigerung der Lebensqualität (LQ) von Menschen mit Demenz gilt als zentrales Ziel der pflegerischen und medizinischen Versorgung. Mit dem QUALIDEM existiert ein theoriebasiertes demenzspezifisches LQ-Instrument in deutscher Sprache. Zur Konstruktvalidität liegen bisher jedoch kaum Ergebnisse vor. Ziel: Evaluation der Itemschwierigkeit, internen Konsistenz und diskriminanten sowie konvergenten Validität des deutschsprachigen QUALIDEM. Methode: Die Überprüfung der Konstruktvalidität erfolgt anhand eines Multi-Trait-One-Method Ansatzes basierend auf einer Stichprobe von 167 Menschen mit einer leichten bis schweren und 71 mit einer sehr schweren Demenz. Ergebnisse: Der Zusammenhang zwischen den QUALIDEM-Subskalen und den Vergleichskonstrukten Agitation, Depression, Angst, Abweichendes motorisches Verhalten, Apathie, Schmerzempfinden und Lebensqualität, erfasst anhand des Alzheimer’s Disease Related Quality of Life Instruments, bestätigte sich in 82 % der vorformulierten Hypothesen in erwarteter Richtung. Der Korrelationskoeffizient zur diskriminanten Validität liegt zwischen 0,04 und −0,60 und für die konvergente Validität zwischen −0,21 und 0,71. Für die Mehrheit der QUALIDEM-Subskalen konnte eine gute interne Konsistenz ermittelt werden. Schlussfolgerungen: Die Ergebnisse der Studie liefern erste Hinweise für die Konstruktvalidität der QUALIDEM-Subskalen sowie zur Weiterentwicklung des Instruments. In zukünftigen Studien sollte die Validität des deutschsprachigen QUALIDEM weiter untersucht werden.
Background Disturbed sleep among people living with dementia in nursing homes is widespread and is associated with diseases and all-cause mortality. This study examined the sleep of people living with dementia from their perspectives in nursing homes and that of the nurses who care for people living with dementia. Methods A qualitative cross-sectional study was conducted. A total of 15 people living with dementia and 15 nurses in 11 German nursing homes were enrolled in this study. Data was collected between February and August 2021 through semistructured interviews, which were audio recorded and transcribed. Thematic analyses were performed by three independent researchers. Thematic mind maps and controversial findings were discussed with the Research Working Group of People with Dementia of the German Alzheimer Association. Results Thematic analysis identified five overarching themes from the nursing home participants regarding sleep patterns: (1) characteristics of good sleep, (2) characteristics of bad sleep, (3) personal influences of people living with dementia on sleep, (4) environmental factors on sleep, and (5) sleep strategies of people living with dementia. Analysis also identified five overarching themes from the nurses participants: (1) characteristics of good sleep, (2) characteristics of bad sleep, (3) personal influences on sleep, (4) environmental factors on sleep, and (5) interventions for sleep promotion. Conclusions The thematic analyses demonstrated that the perspectives of people living with dementia and nurses indicate the need to give more consideration to psychosocial factors and individual aspects of sleep in clinical practice. The results could also be helpful for the development of targeted assessment instruments and complex non-pharmacological interventions to promote sleep.
Zusammenfassung: Hintergrund: Im Verlauf einer Demenz entwickeln die meisten Betroffenen neuropsychiatrische Symptome. Bisher gibt es kaum Untersuchungen zu den spezifischen Ausprägungsformen dieser in der stationären Langzeitpflege. Fragestellung/Ziel: Differenzierte Untersuchung der Prävalenz und Ausprägungsformen von neuropsychiatrischen Symptomen bei Menschen mit Demenz in der stationären Langzeitpflege. Methoden: Durch eine Sekundäranalyse von Querschnittsdaten der Forschungsprojekte LebenQD I & II sowie FallDem wurde die Prävalenz von neuropsychiatrischen Symptomen bei Menschen mit Demenz in der stationären Langzeitpflege untersucht. Diese wurden mit dem Neuropsychiatrischen Inventar – Version Pflegeheim erhoben. In die Analyse sind Daten von 699 Menschen mit Demenz aus insgesamt 21 Langzeitpflegeeinrichtungen in Nordrhein-Westfalen eingeflossen. Ergebnisse: Die höchste Prävalenz zeigen die Symptome Erregung/Aggression (36%), Depression/Dysphorie (33%), Apathie/Gleichgültigkeit (33%), Reizbarkeit/Labilität (30%) und Abweichendes motorisches Verhalten (27%). Die Symptome Halluzination (9%) und Euphorie/Hochstimmung (6%) haben die geringste Prävalenz. Schlussfolgerungen: Die hohe Prävalenz spezifischer neuropsychiatrischer Symptome und deren Ausprägungsformen bei Menschen mit Demenz verdeutlicht den Bedarf pflegerischer- oder psychosozialer Interventionen, um den Gründen für das Auftreten der Symptome entgegenzuwirken.
