Within the facial trauma field of study, injuries from firearm projectiles are considered the second most common death cause, surpassed only by car accidents. Such injuries can result in comminuted fractures causing great substance loss and making it rather difficult to reconstruct the affected site. Furthermore, tissue fragments, the projectile itself or affected surface can be displaced to other regions in the oral cavity. The imaging exams, like computed tomography, are essential for determining fractures or foreign bodies. Any fragments as well as unviable tissues and teeth should be carefully removed, by conservative debridement and wound repairs. This study aims to report two cases of patients wounded by firearm projectiles in which teeth fragments were displaced to other areas of the oral cavity. They were removed later on in an outpatient clinic.
O fibroma ossificante (FO) é uma neoplasia fibro-óssea benigna da região craniofacial de origem odontogênica, formado a partir de células mesenquimais multipotentes do ligamento periodontal, as quais são capazes de formar osso, tecido fibroso e cemento. Acredita-se que exodontias prévias, infecções, trauma ou uma perturbação de origem congênita na maturação óssea poderiam servir como fatores predisponentes para o desenvolvimento do FO. Radiograficamente, as lesões de FO iniciais são representadas por uma imagem radiolúcida, unilocular, redonda ou oval, de margem bem circunscrita. No estágio tardio, o componente mineralizado é circundado por uma fina cápsula fibrosa representada por uma linha radiolúcida delgada, que envolve toda a lesão. O objetivo deste trabalho é discutir a abordagem cirúrgica de um FO localizado em região atípica da mandíbula. Relato de caso: paciente leucoderma, 18 anos de idade, sexo feminino, cursando com aumento de volume em região posterior mandibular direita. No exame de imagem, identificou-se uma lesão mista, bem definida, não corticalizada, medindo aproximadamente 3 cm x 2,5 cm. Após realização de biópsia incisional, confirmou-se o diagnóstico de FO. Sendo assim, optou-se pela curetagem acompanhada de uma osteotomia periférica da lesão e reabilitação da região com enxerto ósseo liofilizado, além da instalação de uma placa de reconstrução na base da mandíbula do sistema 2.4 mm. Considerações finais: É necessário ter conhecimento sobre as lesões de aspecto radiográfico misto, que podem fazer diagnóstico diferencial com o FO, para que possa ser realizada uma correta intervenção, visto que para cada lesão há uma abordagem diferente.
<p class="Body"><strong>Aim:</strong> to report on the applicability of biomodels obtained from computerized tomography (CT) images through the technique of stereolithography (SLA) and three-dimensional printing (3DP), in the clinical case reports of patients who underwent surgeries involving dentoskeletal deformity, oral pathology and oral and maxillofacial trauma. <strong>Metodology: </strong> Clinical case 01 deals with fractures in the orbital and zygomatic arch region that required reconstruction and correction of diplopia, by means of CT images, using the technique of three-dimensional printing through SLA, generating a mirrored biomodel for surgical planning and the making of customized prostheses. In clinical case 02, by means of CT images, a biomodel utilizing the 3DP technique showed the total area invaded by invasive ameloblastoma, making it possible to plan the osteotomy with maximum preservation of the adjacent tissue and prior modeling of the plate. In clinical case 03, rapid prototyping technology (RP) was used to make customized prostheses for temporomandibular articulation, with the goal of correcting a serious idiopathic pathology provoking the resorption of the right and left condyles. <strong>Discussion: </strong>Complex cases require the team to have recourse to technology for the implementation of the procedures, in order to offer excellent quality treatment to the patient, in addition to facilitating surgical planning and permitting the construction of customized prostheses. <strong>Conclusion:</strong> that rapid prototyping for the acquisition of biomodels is an important auxiliary tool for the surgical team. </p>
ResumoFerimentos por Projétil de Arma de Fogo (FPAF) são considerados, dentro do segmento trauma facial, o segundo colocado em causas de morte, sendo superado apenas pelos acidentes automobilísticos. Ferimentos que possuem essa característica, causam fraturas cominutivas e com grande perda de substância dificultando a reconstrução do local afetado. Além disso, fragmentos resultantes, sejam do projétil ou da superfície do tecido afetado, podem ser deslocados para outras regiões sendo um fator a mais de atenção durante a avaliação. Os exames de imagem, como a tomografia computadorizada, são fundamentais para determinar a presença de fraturas ou de corpos estranhos. Dessa forma, os fragmentos deveriam ser removidos cuidadosamente bem como dentes e tecidos inviáveis, seguido de debridamento conservador e fechamento da ferida. O presente trabalho temo como objetivo, relatar dois casos de pacientes acometidos por projétil de arma de fogo, cujos fragmentos dentários foram deslocados para outros espaços da cavidade oral, com sua remoção realizada em âmbito ambulatorial tardiamente, abordando seus aspectos técnicos.
