The purpose of this study was to compare the efficacy and tolerability of venlafaxine and amitriptyline in outpatients with major depression with or without melancholia. This was an 8-week, multicentre, randomized, double-blind, parallel-group comparison of venlafaxine and amitriptyline. Outpatients with DSM-IV major depression, a minimum score of 20 on the 21-item Hamilton Depression Rating Scale (HAM-D), and depressive symptoms for at least 1 month were eligible. Patients were randomly assigned to venlafaxine or amitriptyline, both drugs titrated to a maximum of 150 mg/day until study day 15. The primary efficacy variables were the final on-therapy scores on the HAM-D, Montgomery-Asberg Depression Rating Scale and Clinical Global Impression severity scales. Data were evaluated on an intent-to-treat basis using the LOCF method. One hundred and 16 patients were randomized, and 115 were evaluated for efficacy. Both drugs showed efficacy in the treatment of depression with or without melancholia. No significant differences were noted between treatments for any efficacy parameter. However, significantly (p < 0.05) more patients in the amitriptyline group had at least one adverse event. These results should support the efficacy and tolerability of venlafaxine in comparison with amitriptyline for treating major depression with or without melancholia.

ResumoContexto: Os conceitos de transtornos de humor e de personalidade sofreram diversas mudanças nas últimas décadas. Historicamente, esses conceitos foram construídos em paralelo, isto é, transtornos de humor e personalidade são considerados como transtornos independentes em relação ao diagnóstico, ao prognóstico e ao tratamento. Recentemente, entretanto, novas propostas na conceituação nosológica desses transtornos levantaram a questão de uma possível sobreposição dessas entidades, tornando o diagnóstico diferencial entre esses transtornos muitas vezes difícil. Objetivos: Realizar uma revisão de literatura sobre a evolução dos conceitos de transtorno de humor e personalidade sob uma perspectiva histórica, focando em publicações relacionadas ao diagnóstico. Método: Revisão compreensiva da literatura utilizando o banco de dados MEDLINE (1990MEDLINE ( -2007. Resultados: Ao contrário do conceito de transtorno de humor, que se apresenta estável e relativamente sem modificações desde suas primeiras descrições, o conceito de transtorno de personalidade mostra considerável variação ao longo dos diferentes achados de literatura. Assim, ambos os grupos de transtornos têm sido conceituados tanto categorial como dimensionalmente. Esta última abordagem pode ser, em parte, responsável pelas dificuldades algumas vezes encontradas no diagnóstico diferencial desses transtornos. Conclusão: O diagnóstico diferencial entre transtornos de humor e personalidade ainda representa um sério problema na prática clínica e não pode ser completamente esclarecido com base nas evidências disponíveis. Oral paliperidone ER is a rational treatment alternative for patients with schizophrenia whose antipsychotic regimens are switched because of unsuccessful treatment with oral risperidone according to individual clinical judgment. Study limitations included the open-label study design, lack of placebo, and use of subjective clinical judgment to determine lack of efficacy, intolerability, or nonadherence with oral risperidone.

Introduction: Aripiprazole, a dopamine D2 receptor partial agonist, has also partial agonist activity at serotonin (5-HT)1A receptors and antagonist activity at 5-HT2A receptors.Methods: In this 8-week, multicenter, randomized, parallel-group, open-label, flexible-dose study, patients diagnosed with schizophrenia or schizoaffective disorder were randomized to aripiprazole 15–30 mg/day or haloperidol 10–15 mg/day.Results: Patients treated with both aripiprazole and haloperidol improved from baseline in Positive and Negative Syndrome Scale total, positive, and negative scores as well as in Clinical Global Impressions scores (all P<.001). At the end of the study, the percentage of patients classified as responders—according to ≥40% reduction in the Positive and Negative Syndrome Scale negative subscale score—was significantly higher in the aripiprazole group (20%) than in the haloperidol group (0%) (P<.05). Additionally, a higher number of patients receiving haloperidol required more anticholinergic medications (P<.001) than aripiprazole-treated patients, whereas more aripiprazole (45.5%) than haloperidol-treated patients (12.9%) required benzodiazepines (P=.002). At endpoint, rates of preference of medication were higher in the aripiprazole group (63.2%) than in the haloperidol group (21.7%), as expressed by patients and caregivers (P=.001).Conclusion: Aripiprazole and haloperidol had similar efficacy in terms of reduction of overall psychopathology. Although aripiprazole has been demonstrated to be superior concerning negative symptoms and in terms of tolerability (extrapyramidal symptoms) and preferred by patients and caregivers than haloperidol, significantly more aripiprazole-treated patients required benzodiazepines.
