Introduction: Coronavirus disease 2019 (COVID-19) predominantly affects the respiratory system, but there have been many reports of skin involvement. Several cutaneous manifestations associated with COVID-19 have been identified, including acral lesions (pernio-like), which we intend to better characterize in this study. Methods: Demographic, clinical and major histopathologic characteristics of acral skin lesions occurring during the acute infection stage of COVID-19 disease were evaluated in this survey conducted in the Dermatology Division of Santa Casa Hospital Complex, Federal University of Health Sciences, Porto Alegre, Brazil. Results: In the present study, we evaluated eight female patients with acute SARS-CoV-2 infection confirmed by PCR with acral skin lesions including pernio-like. The mean age was 43.6 and with no other acute disease, who had a good outcome with need for hospitalization only in 2 cases. Pernio-like lesions were observed under hot weather conditions in patients who denied any previous history of similar cutaneous manifestations. Some acral lesions resembled those of lupus erythematosus and of herpes simplex paronychia, but laboratory tests were negative for those conditions. Unusual nail involvement was present, characterized by half-moon shaped erythematous bands at the distal margin of the lunula. Conclusion: Recognition of these skin lesions is important in that they could be potential markers, aiding in the diagnosis and prognosis of COVID-19 disease.
INTRODUÇÃO: Síndrome de McCune-Albright é uma doença genética causada por mutações somáticas no gene pós-zigótico GNAS1, caracterizada por puberdade precoce, manchas café-com-leite e displasia fibrosa óssea 1,2. A prevalência estimada varia entre 1/100.000 e 1/1.000.0003. O fenótipo clínico varia consideravelmente, mas puberdade precoce é a manifestação mais comumente relatada 4. MÉTODOS: Trata-se de um relato de caso. RELATO DO CASO: Paciente feminina, 2 anos, trazida ao CRAI do Hospital Materno Infantil Presidente Vargas (HMIPV) por sangramento vaginal, devido a hipótese de abuso sexual. Ao exame físico, apresentava presença de sangue em pequena quantidade fluindo pelo orifício vaginal, vulva com sinais de estrogenização (mucosa trófica), hímen íntegro com bordas aumentadas de volume e telarca, ambos sugestivos de estímulo hormonal. À inspeção da pele percebeu-se mancha hiperpigmentada na nuca. Abdome indolor a palpação, demais sem particularidades. Solicitados exames para averiguar a causa da provável puberdade precoce e excluir hipótese de abuso. Os exames laboratoriais apresentaram-se dentro dos parâmetros esperados para a idade (LH 0,1; FSH 0,2; prolactina 20,3), à exceção da dosagem do estradiol, de resultado 32. À ecografia pélvica exibia: útero homogêneo medindo 3,5 cm 3 , endométrio de 0,1cm, ovário esquerdo 0,4cm 3 e ovário direito com cisto com dimensões 1,7cm x 1,7cm x 1,4cm. O exame de ressonância magnética cerebral teve resultado normal, sem nenhuma alteração digna de nota. A radiografia de mão e punho para averiguar idade óssea teve como resultado um desvio padrão de 4 meses, porém, em exames subsequentes apresentou desvios padrão em curva ascendente. A cintilografia óssea apresentava focos de hipercaptação nas regiões frontal e occipital do crânio, além da região da mandíbula. A
