The Finnish health and social care sector is currently undergoing numerous reforms. These reforms involve novel demands regarding the role of steering, decision-making, and management in health and social services. This article sheds light on some of the critical factors encountered by decision-makers and managers when implementing reforms to change health and social services. The article investigates cooperation between the steering ministries and different dimensions of management in local health and social service organizations. Interview data from the ministries were studied through content analysis, and quantitative survey data were analyzed using mean values guided by the model of multidimensional management. The co-operation between different ministries has intensified while implementing different reforms, but functional and cultural boundaries between them persist. The management dimensions in changing health and social service organizations stress the role of managers as facilitators and enablers, highlighting the significance of caring for the human resource dimension as one of the managers' core tasks. The successful implementation of change should never be automatic. What emerges prominently is the skill essential to achieve cooperation which transcends the borders of both multiprofessional and traditional professional and administrative work both in ministries and local health and social service organizations.

Limitations: We had three large organizations in our study. All organizations were in different phases of their development, which may affect our results.
Lainsäädäntö velvoittaa kuntia tarjoamaan asiakkaiden tarpeita vastaavia päihdepalveluita. Tässä katsauksessa tuodaan esille, miten kuntien välillä on silti suuria eroja päihdepalvelujen tarjonnassa. Aineistona käytetään 32 päihdetyön ammattilaisen haastatteluja sekä tilastollisia aineistoja. Haastatteluaineisto on analysoitu teoriasidonnaisella sisällönanalyysillä ja tilastoaineistot K-keskiarvon ryhmittelyanalyysillä sekä erotteluanalyysillä. Tilastoaineiston analyysit paljastavat, että etenkin syrjäisissä vähenevän väestön kunnissa painottuu päihdepotilaiden vuodeosastohoito ja päihdehuollon avopalvelujen asiakasmäärät ovat pieniä. Haastatteluaineisto tuo esille, että ammattilaiset tiedostavat lainsäädännön velvoitteet mutta niukat resurssit ja taloudelliset syyt estävät tarvelähtöisten palvelujen tarjoamisen. Lainsäädännön velvoitteet eivät siis täyty kaikkialla, ja on vaikea sanoa, tuoko tulevaisuuskaan parannuksia tullessaan.
Artikkelissa tarkastellaan luottamusta kontekstina laaja organisaatiomuutos, jossa usean kunnan sosiaali- ja terveyspalvelut yhdistettiin kuntayhtymäksi. Haastatteluaineisto kerättiin eri tasojen johtajilta sekä työntekijöiltä eri toimialoilta ja analysoitiin sisällönanalyysillä. Johtajiin sekä organisaatioon kohdistuneen luottamuksen muutosta tarkasteltiin luotettavuuden osatekijöillä, jotka vaikuttavat luottamuksen syntyyn. Luottamus sujuvoittaa yhdessä työskentelyä, edistää työtyytyväisyyttä ja edesauttaa organisaatiomuutoksen onnistumista, joten organisaatioissa kannattaa kiinnittää siihen huomiota. Muutokset johtamis- ja organisaatiorakenteessa, toimintakulttuurissa sekä osallisuuden kokemuksissa heikensivät johtajien ja uuden organisaation kokemista luotettavana. Muun muassa kuntayhtymän kehittämisen terveydenhoitokeskeisyys vaikutti tähän. Vastaavanlaisissa muutoksissa kannattaa kiinnittää enemmän huomiota muutosviestintään, työntekijöiden ja johtajien tukemiseen sekä työntekijöiden ja eri johtotasojen väliseen vuorovaikutukseen.
