ZusammenfassungBei einer Notfall-Endoskopie eines 45-jährigen Mannes mit einer komplizierten medizinischen Vorgeschichte wurde im Magenkorpus ein kleines Ulkus in einer ca. 4 cm durchmessenden, halbkugeligen Impression gefunden. Wir entschlossen uns zu einer sofortigen Durchführung einer Angio-CT-Untersuchung von Abdomen und Thorax. Impression und Ulkus wurden durch ein teilthrombosiertes Aneurysma der Arteria lienalis verursacht. Dieses wurde sofort angiografisch mittels perkutaner transluminaler Implantation eines gecoverten Stents ausgeschaltet. Der Patient erholte sich und konnte nach 12 Tagen aus dem Krankenhaus entlassen werden. Eine Literatursuche ergab, dass in solchen Fällen der sofortige Wechsel von Endoskopie zu einem gefäßchirurgischen Eingriff die beste Option darstellt. Endoskopiker müssen die sog. „Herald-Blutung“ als solche erkennen und die weiteren diagnostischen Schritte sofort veranlassen, da eine zweite Blutung meist fatal endet. Dadurch kann die Prognose auf 20–90 % Überleben verbessert werden. Leider fühlten sich in einer aktuellen Umfrage 50 % der Gastroenterologen über diese Tatsache nicht gut informiert und hätten wahrscheinlich nicht die korrekten Entscheidungen getroffen.
