Background: Many interventions found to be effective in health services research studies fail to translate into meaningful patient care outcomes across multiple contexts. Health services researchers recognize the need to evaluate not only summative outcomes but also formative outcomes to assess the extent to which implementation is effective in a specific setting, prolongs sustainability, and promotes dissemination into other settings. Many implementation theories have been published to help promote effective implementation. However, they overlap considerably in the constructs included in individual theories, and a comparison of theories reveals that each is missing important constructs included in other theories. In addition, terminology and definitions are not consistent across theories. We describe the Consolidated Framework For Implementation Research (CFIR) that offers an overarching typology to promote implementation theory development and verification about what works where and why across multiple contexts.
Background & Aims-Data on secular trends and outcomes of eosinophilic esophagitis (EE) are scarce. We performed a population-based study to assess the epidemiology and outcomes of EE in Olmsted County, Minnesota, over the last 3 decades.
Midesophageal biopsies from normal-appearing mucosa should be obtained in all patients with unexplained solid food dysphagia; this may diagnose EE in about one in 10 cases.
Children's experiences and voices are underrepresented in academic literature and professional practice around domestic violence and abuse. The project 'Understanding Agency and Resistance Strategies' addresses this absence, through direct engagement with children. We present an analysis from interviews with 21 children in the United Kingdom (12 girls and 9 boys, aged 8-18 years), about their experiences of domestic violence and abuse, and their responses to this violence. These interviews were analysed using interpretive interactionism. Three themes from this analysis are presented: a) 'Children's experiences of abusive control', which explores children's awareness of controlling behaviour by the adult perpetrator, their experience of that control, and its impact on them; b) 'Constraint', which explores how children experience the constraint associated with coercive control in situations of domestic violence, and c) 'Children as agents' which explores children's strategies for managing controlling behaviour in their home and in family relationships. The paper argues that, in situations where violence and abuse occurs between adult intimate partners, children are significantly impacted, and can be reasonably described as victims of abusive control. Recognising children as direct victims of domestic violence and abuse would produce significant changes in the way professionals respond to them, by 1) recognising children's experience of the impact of domestic violence and abuse; 2) recognising children's agency, undermining the perception of them as passive 'witnesses' or 'collateral damage' in adult abusive encounters; and 3) strengthening professional responses to them as direct victims, not as passive witnesses to violence.
The creative research method 'draw and write' has been used in health, social care and education research for several decades. A literature search of studies utilising this method was conducted during the planning stages of a study exploring primary school children's perceptions of infant feeding. A review of this literature noted a range of benefits of 'draw and write' in enabling child participation. However, it also identified that the method has been used inconsistently, and found that there are issues for researchers in relation to interpretation of creative work and analysis of data. As a result of this an improvement on this method, entitled 'draw, write and tell', was developed in an attempt to provide a more child orientated and consistent approach to data collection, interpretation and analysis. This article identifies the issues relating to 'draw and write' and describes the development and application of 'draw, write and tell' as a case study, noting its limitations and benefits. Previous research has demonstrated that many people have misconceptions about basic properties of motion. In two experiments, we examined whether people are more likely to produce dynamically correct predictions about basic motion problems involving situations with which they are familiar, and whether solving such problems enhances performance on a subsequent abstract problem. In Experiment 1, college students were asked to predict the trajectories of objects exiting a curved tube. Subjects were more accurate on the familiar version of the problem, and there was no evidence oftransfer to the abstract problem. In Experiment 2, two familiar problems were provided in an attempt to enhance subjects' tendency to extract the general structure of the problems. Once again, they gave more correct responses to the familiar problems but failed to generalize to the abstract problem. Formal physics training was associated with correct predictions for the abstract problem but was unrelated to performance on the familiar problems.
The fitness of non-feeding adult insects depends on energy accumulated during the larval stage. Larvae of the caddisfly Asynarchus nigriculus primarily feed on plant detritus, but supplement their diet with animal material obtained through cannibalism. Habitat drying constrains development in many populations of this species, and we hypothesized that cannibalism should accelerate development to facilitate timely metamorphosis. We manipulated larval diets in a field experiment by supplementing detritus with animal material, and in a laboratory experiment by varying animal material and detritus quality (conditioned vs unconditioned). We measured the effects of dietary manipulation on larval and pupal growth and development, the timing of metamorphosis, and adult fitness correlates. The results of the laboratory experiment suggest that this species can metamorphose with a detritus-only diet, but development is extremely protracted. In the field experiment, individuals with animal material in their diet had higher larval survival, shorter larval and pupal development times, and earlier emergence dates (7-10 days), than those without a supplement. This delay in emergence should have important effects on survival in natural populations where the difference between desiccation and successful emergence can be only a few days. Dietary supplementation also affected adult body mass (30-40% increase), female fecundity (30% more eggs), and proportional allocation to different adult body parts. Our results are consistent with recent growth-development models that predict coupled (earlier emergence and larger adults) rather than tradeoff responses (earlier emergence and smaller adults) to pre-threshold manipulation of larval diets. Many detritivorous aquatic insects supplement their diets with animal material, and our data provide evidence that this supplementation can have strong effects on fitness. This type of dietary supplementation should be especially important for taxa that do not feed as adults, and in temporary habitats that impose time constraints on larval development.
