El leiomioma parauretral es una neoplasia benigna con pocos casos descritos en la literatura. El leiomioma abarca el 5% de los casos de las masas parauretrales, ocupando el cuarto lugar como una de las causas menos frecuentes, después del divertículo parauretral. Se presenta el caso de una paciente de 43 años con antecedente de infección urinaria a repetición, con una masa de 8 cm en pared vaginal anterior de cuatro años de evolución. La cistoscopia fue normal y la ecografía mostró una lesión sólida parauretral. Se realizó resección por colpotomía sin complicaciones, con resultado de patología de leiomioma. Control postquirúrgico y uroflujometría normal. Dentro de las posibilidades diagnósticas ante una masa parauretral, el leiomioma debe considerarse. La sintomatología es variable, siendo los síntomas urinarios los más frecuentes. La ecografía y la resonancia son herramientas fundamentales y el manejo se basa en la resección quirúrgica, generalmente vía vaginal.
INTRODUCCIÓN Las enfermedades pulmonares intersticiales plantean un importante reto diagnóstico en pediatría; son un grupo heterogéneo con características radiológicas, histológicas y clínicas comunes y tienen tasas elevadas de morbilidad y mortalidad. Se caracterizan por presentar cambios inflamatorios y fibróticos en la pared alveolar. Entre las diversas causas conocidas se incluyen infecciones, enfermedades autoinmunes, efectos tóxicos de los fármacos y enfermedades con implicación genética establecida, pero queda una gran proporción de casos en los que no se logra conocer la causa.OBJETIVOS Describir un caso de insuficiencia respiratoria de instauración rápida secundaria a fibrosis pulmonar de causa desconocida en un adolescente inicial, con miras al estudio y reconocimiento de estas afecciones por parte del personal médico.DESCRIPCIÓN DEL CASO Adolescente inicial sin antecedentes de importancia quien ingresó con cuadro de dificultad respiratoria interpretado inicialmente como de naturaleza infecciosa, con evolución hacia el deterioro progresivo con hipoxemia marcada, hipertensión pulmonar y deterioro radiológico que, a pesar de múltiples estrategias ventilatorias, no se logró corregir. Falleció después de 44 días con parámetros ventilatorios progresivamente altos, sin respuesta a ninguno de los tratamientos instaurados. Para aclarar la etiología se hicieron biopsia pulmonar, tinciones especiales, marcaciones de inmunohistoquística, estudios sistémicos y por imágenes; en la autopsia se concluyó que la muerte había sido secundaria a falla ventilatoria por alteración del índice ventilación-perfusión originada en una fibrosis intersticial pulmonar en estado terminal, sin que fuera posible determinar la causa primaria.DISCUSIÓN La fibrosis intersticial idiopática es una condición importante de difícil diagnóstico en pediatría, con un pronóstico extremadamente malo en la mayoría de los pacientes; se informa un caso de insuficiencia rápidamente progresiva secundaria a esta condición en el que se aplicaron todas las medidas establecidas en nuestro medio y se recurrió incluso a la asesoría internacional para analizar los hallazgos histopatológicos, sin obtener una respuesta positiva.
