A review in a half-stay hospital ABSTRACTChronic wounds are a significant health problem. Electrical stimulation seems to produce a significantly greater reduction in surface area and more complete healing of difficult-to-heal and poorly healing ulcers compared to standard therapy without compressive bandaging. Objectives: To evaluate the effect that radiofrequency at low intensity and with non-thermal effects has on the different components of the mechanism of the healing process. Methodology: A tecartherapy device (CAPENERGY C200) was used for the treatment. A total of 10 radiofrequency sessions were applied once a week with a power of 60% and a frequency of 1.2 MHz for 30 minutes. Results: The presence of oedema, observed in all patients in the lower extremity region, disappeared in 30 of the 36 patients (Wilcoxon p = 0.004). This result was confirmed by ultrasound. The mean subcutaneous cellular oedema decreased by 1.73 cm (Friedman p = 0.000). The temperature of the area taken before and after treatment increased by an average of 1.4 °C. These differences are statistically significant (Wilcoxon p = 0.000). Conclusions: Radiofrequency appears to be able to reduce the long healing process of torpidly evolving lesions, and we found significant differences throughout the treatment and a progressive reduction in lesion measurements and faster healing of complex wounds.
The new perspectives on medical sciences and the relationship health-disease, under the light of advanced sciences that perceive the worls as a complex and chaotic phenomenon, demand a change of thought and attitudes for health professionals, particularly nurses. In this work, some concepts such as chaos, complexity and fractality are defined, and the nursing role is separated from euclidian geometry where everything is structurally shaped, to be modified towards a more complex perception where all the parts make the whole that eventually conforms health; health should be understood as a multifactorial system, in order to cope with the challenges that new technologies offer and modify nurses' future behaviour.Key words: complexity, chaos, multifactorial system, fractality. RESUMENLa nueva visión de las ciencias médicas y de la relación salud-enfermedad vista a la luz de las ciencias avanzadas que perciben al mundo como un fenómeno complejo y caótico, demanda al trabajador de salud y en particular a la enfermera, un cambio en la forma de pensar y actuar. En este trabajo, se definen algunos de los conceptos de caos, complejidad, fractalidad y se involucra el papel de la enfermera ante esta forma de ver la ciencia, separándose de la geometría euclidiana, donde todo es de formas estructuradas para modificarlo por una percepción más compleja, donde cada una de las partes hacen el todo que finalmente conforma la salud, entendiendo ésta por un sistema multifactorial que debe ser comprendido de esta manera para poder afrontar los retos que las nuevas tecnologías le plantean al personal de enfermería modificando así, su conducta futura.Palabras clave: complejidad, caos, sistema multifactorial, fractalidad. INTRODUCCIÓNEn la actualidad, el personal que atiende de manera directa al enfermo y que se encuentra vinculado estrechamente a su salud y a su enfermedad, está plagado, por no decir pletórico o inundado del pensamiento filosófico que ha regido a las ciencias físicas durante los últimos siglos, es decir, un pensamiento mecanicista y positivista. El binomio causa -efecto, ha predominado en el quehacer médico diario. La medicina que para unos es una ciencia, para otros es un arte (Ierodiakonou, 1993), sin embargo, lo que tiene que estar claro independientemente de esta polémica es que las ciencias médicas deben asumir un nuevo papel bajo la ópti-ca de las nuevas concepciones teóricas alrededor del comportamiento de la naturaleza y la biología.
Radiofrequency assisted medical devices have evolved dramatically in the last two decades, such that a simple set of fairly basic tools has now become an extremely sophisticated option. Most importantly, a physician’s understanding of these devices can maximize treatment results. Diathermy, meaning “through heat,” consists in the application of short-wave electromagnetic energy. The factor that determines whether a diathermy machine will increase body temperature is the amount of energy absorbed by the tissue. The development of this device allows selecting the frequency to apply between 0.8 1 and 1.2 MHz; the application of high energy power, through several channels and increasing the treatment area, allows taking the radiofrequency to another level in rehabilitation sports medicine. RF is applied to the target area by a handpiece that delivers energy, while a grounding pad is placed elsewhere on the body for the energy to pass through. The entry of RF into the body at the site of contact with the active electrode leads to heating a volume of tissue. This ability to heat a volume of tissue in a non-invasive way produces the immediate contraction of collagen, and the delayed synthesis of collagen, by thermal induction fibroblasts, and the production of epithelial, vascular growth factors. RF has a firm, safe and increasingly popular place in the therapeutic arsenal of sports medicine.
Introducción: En el tratamiento de las lesiones deportivas se utilizan diferentes formas de energía electromagnética, entre ellas la diatermia, con fines terapéuticos. El uso seguro de los dispositivos de diatermia, tanto para el paciente como para el personal sanitario que los aplica, se basa en estándares profesionales, códigos de seguridad y directrices varias, que determinan los procedimientos de uso seguros. Estas recomendaciones se encuentran dispersas y no son bien conocidas por los usuarios, por lo que fue el objetivo de esta revisión el sintetizar y actualizar dichas recomendaciones. Metodología: revisión de la normativa vigente en España, sobre el uso de los equipos médicos de Diatermia. Discusión: Esta falta de claridad en algunos fabricantes hace que se cuestione la usabilidad o aplicación de los productos que se fabrican para uso sanitario. Esta situación favorece un marketing engañoso y el cliente final queda no solo a merced de una adquisición de un equipo sin la certificación médica adecuada, sino además desprotegido legalmente en su aplicación, lo que puede traer como consecuencia sanciones legales.
