ZusammenfassungDas funktionseingeschränkte Handgelenk nach distaler Radiusfraktur stellt eine Spätkomplikation dar, die zur Arbeitsunfähigkeit führen kann. An 126 Patienten mit stattgehabter distaler Radiusfraktur wurde die Wertigkeit der stationären Ergo-und Physiotherapie im Sinne der Berufsgenossenschaftlichen stationären Weiterbehandlung (BGSW) auf die Funktion des Handgelenkes und die Wiedererlangung der Arbeitsfähigkeit retrospektiv überprüft. Im Median betrug die Arbeitsunfähigkeit zwischen Unfall und stationärer Aufnahme 96 Tage (Spanne 29 ± 1507 Tage), die Dauer der stationären BGSW 22 Tage (2 ± 56 Tage) und die Zeit nach BGSW bis zur Arbeitsfähigkeit 4 Tage. 93% der berufstätigen Patienten konnten durch die BGSW die Arbeitsfähigkeit wiedererlangen. Die Funktion des Handgelenkes konnte in der Dorsalflexion um 15 128 (Mittelwert SD), in der Palmarflexion um 10 128, in der Radialduktion um 6 98, und in der Ulnarduktion um 7 98 sowie in der Supination um 23 248 und der Pronation um 9 208 verbessert werden (sämtlich p < 0,0001), wobei für alle Bewegungsrichtungen bei schlechterer Ausgangsfunktion ein gröûerer Bewegungszugewinn erreicht werden konnte. Der Zugewinn war unabhängig von dem Zeitintervall zwischen Unfall und BGSW, Art der primären Versorgung oder posttraumatischem radiologischen Befund. Bei längerer Arbeitsunfähigkeit (> 3 Monate) nach distaler Radiusfraktur sollte eine Behandlung im Sinne der BGSW erwogen werden. AbstractImpaired wrist function after fracture of the distal radius is a late complication and may lead to disability. The value of hospitalbased physical and occupational therapy under the auspice of the employers liability insurance (BGSW) was evaluated in 126 patients after healed fracture of the distal radius. Gain of motion and return to work was monitored. Median time from injury to admission was 96 days (range 29 ± 1507 days), duration of treatment 22 days (range 2 ± 56 days) and return to work 4 days. Return to work was achieved in 93% of the patients. Mean gain of motion in dorsiflexion was 15 128 (mean SD), in palmarflexion 10 128, in radialflexion 6 98, in ulnarflexion 7 98, in supination 23 248, and in pronation 9 208 (all p < 0.0001). The increase was more pronounced in those patients with poor initial function. No correlation was seen between time until BGSW treatment, initial treatment, and posttraumatic radiological findings. Hospital-based physical and occupational therapy (BGSW) is recommended for loss of function after radial fracture at latest 3 month postinjury.
