La presente obra se hace una revisión sistemática de las patologías más comunes en el primer nivel de atención y su manejo clínico. Se incluyen también algunos casos relevantes para el estudio del médico en formación y personal de salud en general.
El presente texto está dirigido tanto a estudiantes de medicina como a médicos generales, el lector encontrará los fundamentos sobre Cirugía General. De manera que la obra sirve como guía importante y práctica, que todo profesional de la salud enfocado en la cirugía debe tener en su biblioteca.
La Cirugía General Digestiva en ciertos países es considerada una especialidad más y se entiende por super especialización la profundización en una de sus ramas quirúrgicas. En este sentido, se considera una especialidad médica dedicada al estudio, diagnóstico y tratamiento de las enfermedades cuyo tratamiento requiere generalmente de la realización de una intervención quirúrgica, sea ésta programada o de urgencia; y cuyo origen sea benigno, traumático o relacionado con un cáncer. Comprende el esófago, estómago, vesícula y vía biliar, páncreas, hígado, bazo, intestino, colon, recto y ano. Es decir, su campo de intervención de clase quirúrgica abarca: El aparato digestivo, incluyendo el tracto gastrointestinal y el sistema hepato-bilio-pancreático El sistema endocrino, incluyendo las glándulas suprarrenales, tiroides, paratiroides, mama y otras glándulas incluidas en el aparato digestivo. Asimismo, aborda la piel y tejidos blandos, afecciones externas de cabeza y cuello, reparación de hernias y eventraciones de la pared abdominal, etc. Algunas de las enfermedades que interesan a la especialidad de Cirugía General y Digestiva, en ocasiones se pueden solapar con otras especialidades médicas en cuanto se refiere a su resolución con una intervención quirúrgica, pero en todo caso el cirujano general es competente para hacerlo, como las más comunes: Apendicitis; Granulomas; Colelitiasis; Hernias de diferentes tipos (hiato, umbilical, inguinal, etc.); Hemorroides; Quistes; Alteraciones de la glándula tiroides; Obstrucción u oclusión intestinal, Adherencias. Para detectar la existencia de estas patologías que afecten al sistema venoso y arterial, existen diferentes pruebas diagnósticas que se pueden realizar. En el ámbito de la Radiología, la radiografía, el TAC (con o sin contraste) o la resonancia magnética. Estas pruebas diagnósticas, pueden aportan información suficiente para el diagnóstico. Asimismo, la Biopsia, Ecografía, Colonoscopia, Endoscopia gastrointestinal superior, Test del aliento. Al tratarse de cirugía, todos los tratamientos son intervencionistas, utilizándose diferentes técnicas quirúrgicas en función del tipo de patología y de la gravedad de la misma, entre ellas: la Cirugía ambulatoria, Cirugía laparoscópica (mínimamente invasiva), Cirugía convencional (Cirugía mayor o cirugía abierta). Por Laparoscopía se pueden realizar, por ejemplo: Colecistectomía Laparoscópica por Cálculos o Pólipos Vesiculares, Cirugía Antirreflujo; Gastrectomía Total por Cáncer; Hernias Complejas; Hepatectomías; Anastomosis Biliodigestivas; Cirugía del Cáncer de Vesícula; Cirugía de Tumores Hepáticos y Pancreáticos; Esplenectomía (extirpación del bazo); Extirpación de quistes Hidatídicos Hepáticos; Biopsias de Ganglios Linfáticos Abdominales. En el área de la Proctología o Coloproctología considerada también como una subespecialidad de cirugía general y del aparato digestivo la cual se ocupa del diagnóstico y tratamiento, sea quirúrgico o no, de aquellas enfermedades que afecta al ano, el recto y el colon: hemorroides, fisura y fístula anal, abscesos rectales, estreñimiento, colitis ulcerosa, condilomas, incontinencia fecal, prolapso de recto, divertículos de colon, enfermedad inflamatoria intestinal, pólipos de colon y recto, cáncer colorrectal, etc. Por tanto, la Cirugía de colon – recto, engloba las enfermedades que se localizan en el colon y en recto, y se podrían esquematizar de manera sencilla en: Inflamaciones crónicas del intestino tanto grueso como delgado, tipo colitis ulcerosa, Chron, que no responden a terapia médica. Patología benigna, apéndice, pólipos grandes, diverticulosis, diverticulitis, divertículo de Meckel. Patología maligna que incluye todos los tipos de cáncer de colon y recto. En resumen, la cirugía colorrectal es uno de los grandes campos de la Cirugía General y Digestiva que incluye numerosas indicaciones y técnicas. Dentro de las diferentes patologías que se abordan, la más importante por su trascendencia es el cáncer de colon y recto, que es el cuarto cáncer más frecuente en el mundo. Otras patologías también se tratan mediante este tipo de cirugía, así, dentro de las enfermedades inflamatorias intestinales, la colitis ulcerosa, y la enfermedad de Crohn, un 30% de los pacientes precisan cirugía a los 5 años del diagnóstico y un 80% a los 10 años. Por otra parte, según estudios la diverticulosis colónica afecta al 80% de los pacientes entre 65-70 años, presentando episodios de diverticulitis en un 10-20% de los casos de los que 3,8 por 100000 habitantes/año precisan cirugía por enfermedad diverticular. Para tratar estas patologías se hace necesaria una intervención quirúrgica que en la mayoría de los casos conlleva la realización de resecciones intestinales más o menos amplias, en función de la localización y el tipo de enfermedad. La vía de abordaje de estos procedimientos puede ser tanto cirugía abierta como laparoscópica. Una vez realizada la resección intestinal es necesaria la derivación o la reconstrucción del tránsito digestivo. Así, en función del tipo de patología y del paciente, se tomará la decisión de realizar una estoma para lograr Introducción 24 la evacuación de las heces, la cual puede ser temporal o definitivo, o, siempre que sea posible, la realización de una anastomosis. Las anastomosis forman parte de las técnicas habituales en la cirugía colorrectal, consiste en realizar una unión quirúrgica entre los dos cabos intestinales que quedan tras resecar el tramo afecto. Estos aspectos y otros más son abordados en el presente texto, que hoy tiene en sus manos, denominado CIRUGIA DIGESTIVA. DIAGNÓSTICO Y TRATAMIENTO EN CIRUGÍA COLORRECTA el cual se ha destinado al abordaje en nueve (9) capítulos al conocimiento de aspectos fundamentales de las diferentes enfermedades colorrectales haciendo hincapié en el diagnóstico y el tratamiento quirúrgico. Los capítulos contemplados son: CAPÍTULO I. ENFERMEDADES ANALES BENIGNAS (Hemorroides; Fisura anal; Abscesos y Fístulas anales - Fístulas rectovaginales y rectouretrales-; Sinus pilonidal. Hidrosadenitis. Prurito de ano. CAPÍTULO II. ALTERACIONES FUNCIONALES (Incontinencia fecal; Estreñimiento crónico; Síndrome de obstruccción a la defecación y Dolor pélvico crónico. CAPÍTULO III. ENFERMEDADES INFLAMATORIAS DEL COLON (Colitis ulcerosa; Enfermedad de Crohn; Colitis indeterminada; Enfermedad diverticular; Tumores benignos y Poliposis adenomatosa familiar y cáncer colorrectal. CAPÍTULO IV. CÁNCER (Cáncer colorrectal y de ano). CAPÍTULO V. EXÁMENES MÉDICOS Y PRUEBAS DE IMAGEN; CAPÍTULO VI. EL PERIOPERATORIO; CAPÍTULO VII. COMPLICACIONES POSTOPERATORIAS (Infección de herida; Absceso intraabdominal; Dehiscencia anastomótica; Hemorragia Obstrucción intestinal; Evisceración; Lesiones de las vías urinarias). CAPÍTULO VIII. INFECCIÓN POSTOPERATORIA EN CIRUGÍA COLORRECTAL. TRATAMIENTO ANTIBIÓTICO; y por último, se ha destinado el CAPÍTULO IX a tratar dos importantes URGENCIAS debido a su importancia, el Absceso intraabdominal y la Hemorragia Digestiva Baja (HDB).
