desmodonte, existent que l'orthodontiste peut envisager le déplacement
dentaire provoqué. Dans un souci d'intégrer la notion de
coût/bénéfice/sécurité, l'orthodontiste se doit de
prévenir, de réduire et/ou d'éviter les effets
délétères de ses traitements sur les tissus parodontaux.
L'inflammation gingivale à forte composante hyperplasique, les pertes
d'attache, l'alvéolyse et la résorption radiculaire peuvent
entraîner, en valeur absolue, une réduction des structures
parodontales de soutien. La clé de la prévention de ces
problèmes réside dans le contrôle de plaque, phase trop souvent
bâclée, voire négligée. Le challenge, avant tout traitement
orthodontique, aussi bien chez l'enfant que chez l'adulte, est de changer le
comportement du patient face à sa cavité buccale et à la plaque
bactérienne. L'objectif de cet article est de parcourir la
littérature fondée sur les preuves (revues systématiques,
méta-analyses), d'apporter des éléments de réflexion et des
propositions concrètes pour résoudre définitivement cette phase
essentielle de tout traitement orthodontique qu'est la motivation, phase qui
peut aussi être réalisée par des « auxiliaires de santé ».
