Résumé Nous avons mené une étude exploratoire par entretiens sur la place et les activités des comités hospitaliers d’éthique clinique aux États-Unis et en France. En France, les comités sont rares et sont essentiellement des lieux de discussion et de débat interne. Aux États-Unis, ils sont très répandus et ont un triple rôle institutionnel : intervenir dans des cas difficiles, participer à l’élaboration de lignes directrices, former les professionnels de santé. Après avoir présenté les principaux résultats, nous interprétons cette situation par une approche comparative de certaines caractéristiques de l’histoire du mouvement bioéthique et des règles juridiques encadrant l’exercice de la médecine et de leurs fondements culturels.
Introduction: In France, the government's plan to combat Alzheimer's disease takes into consideration mostly the beginnings of the disease; it does not deal with the sufferers’ end of life. For this pathology, the very idea of a palliative care phase and its definition are not straightforward. Method: The object of this qualitative study was to clarify this idea through a series of interviews with 44 professionals. Results: The study describes three successive phases in the disease trajectory, revealed by changes in the logic of care: the stimulation phase, the chronic palliative care phase, and the terminal palliative phase. Conclusion: Alzheimer's disease has a specific phase of chronic palliative care. It presents, for professionals, several types of problems related to communication, pain, and feeding.
Objectif : L’expérience des personnes ayant été atteintes par un cancer du sein localisé et engagées dans une activité physique après leur traitement, reste peu connue. Issu d’une recherche qualitative par entretiens, cet article vise à mieux la comprendre. Méthode : Nous avons rencontré 23 femmes âgées de 30 à 84 ans choisies au hasard, de professions et milieux sociaux divers environ un an après la fin du traitement. La moitié avait suivi un programme d’éducation thérapeutique du patient (ETP) associant activité physique adaptée (APA) et ateliers diététiques. Une analyse des entretiens biographiques a permis de saisir la logique de leurs discours. Cette étude s’appuie sur la philosophie de la santé et de la normativité de Georges Canguilhem. Résultats : Le programme d’ETP joue un rôle primordial dans la sensibilisation à l’APA et dans les pratiques futures des ex-patientes. Plus fondamentalement, la place et le sens de l’activité physique dans l’après-traitement semblent liés à différents types de profils normatifs. Se dégagent un profil « convaincu » de l’intérêt de l’activité physique et pratiquant en conséquence ; un profil « contraint » dont les normes de vie sont opposées à une pratique ; un profil « autonome » affirmant ses choix et valeurs ; un profil « ignorant » pour lequel l’activité physique n’a aucun rapport avec le cancer. Conclusion : Pour mieux accompagner les femmes dans l’après traitement du cancer du sein et la reprise d’une activité physique, il serait utile de mieux connaître et prendre en compte le profil de normativité, les valeurs et les trajectoires biographiques des personnes.
In this article we highlight the main points of the development of bioethics and ethics committees in France. We argue that the French cultural context of medicine and its current political transformations favor new models of hospital ethics committee and we provide an example of such a model developed at Paris's Institut Curie, the oldest French cancer treatment and research centre.
