Descripción de una serie de 65 pacientes con migraña crónica: patrón clínico y evolución Resumen Objetivos. Evaluar la estrategia terapéutica realizada, analizando la evolución de los pacientes. Analizar aspectos clínicos y epidemiológicos involucrados en el desarrollo y evolución de la enfermedad. Métodos. Se incluyeron en el estudio 65 pacientes (57 mujeres y 8 hombres) con diagnóstico de migraña crónica, 49 de ellos con abuso de medicación, en seguimiento por el Servicio de Neurología entre 2009 y 2014. El tiempo medio de seguimiento fue de 19 meses. Las variables han sido determinadas como frecuencias absolutas y porcentajes. Resultados. La migraña crónica ha resultado ser más frecuente en mujeres (87,7%); el intervalo de edad más frecuente ha sido 41-50 años (26,2%). El 33.8% de los pacientes presentan depresión o ansiedad; el 26,2%, trastorno del sueño; el 23,1%, fibromialgia. El abuso de medicación sintomática tiene lugar en el 75,4% de los pacientes. El tratamiento preventivo más eficaz es la combinación de neuromodulador y antidepresivo (16,9%). La desintoxicación tiene lugar en el 65,3% de los pacientes que abusan de la medicación sintomática. En el 66,2% de los pacientes, la migraña crónica revierte a episódica. Conclusiones. La migraña crónica es a menudo difícil de tratar. El abuso de la medicación analgésica es el factor predisponente de cronificación de migraña y de aparición de cefalea por abuso de medicación (CAM) más importante, por lo que la desintoxicación y un tratamiento adecuado son las medidas primordiales en el manejo de la enfermedad.
