For overhead athletes and, in particular, baseball pitchers, the rates of success and return to play for those who have undergone arthroscopic repair of type II SLAP lesions are poor, ranging from 7% to 62%. The reasons for the poor results and high failure rates in overhead athletes with type II SLAP repairs are multifactorial and are a combination of many factors. These factors include the failure to establish the diagnosis and treat these athletes preoperatively; the inability of the operating surgeon to differentiate normal anatomic variants from pathologic SLAP lesions at the time of surgery; the surgical technique, which may violate the rotator cuff; or the placement of suture anchors, which restricts external rotation and alters overhead throwing mechanics. The proper diagnosis of SLAP lesions can be difficult because SLAP tears rarely occur in isolation and are often associated with other shoulder pathology. A proper history detailing the onset of symptoms and whether there was an acute episode of trauma or a history of repetitive use is critical. It is important to remember that no single physical examination finding is pathognomonic for SLAP tears. When seen in isolation, SLAP tears may mimic impingement syndrome (52%) or even anterior instability (39%). Surgical treatment of type II SLAP lesions should not be undertaken lightly in overhead athletes. If a 3-month rehabilitation period followed by a return to sports over the following 3 months does not allow the athlete to return to his or her preinjury level, diagnostic arthroscopy with SLAP repair is a reasonable option and can yield excellent results using the proper techniques. The technique described in detail in this article and our video can be technically demanding, but with the key points outlined, it can be reproduced and provide excellent results for overhead athletes undergoing SLAP repair. By not violating the rotator cuff, using a mattress configuration and keeping the suture knot away from the articular surface, and by not going anterior to the biceps tendon for repair, external rotation and strength can be preserved, leading to an excellent result with a predictable return to play for overhead athletes.
hyroid eye disease (TED) is an autoimmune condition commonly associated with thyroid dysfunction, which has a clinical presentation ranging from mild disease to irreversible sight-threatening complications. 1 Until recently, medical therapies for TED have been limited largely to systemic glucocorticoids, which are associated with adverse effects. 1 On January 21, 2020, teprotumumab (Tepezza; Horizon Therapeutics) became the first prescription medication to be granted US Food and Drug Administration (FDA) approval for the treatment of TED. 2 Before FDA approval of teprotumumab, the drug's developer, Horizon Therapeutics, launched a multichannel TED awareness campaign, including a website and Facebook community aimed at providing education to patients and caregivers. 3,4 They also launched an early direct-toconsumer advertising (DTCA) campaign shortly after FDA approval of teprotumumab, which expanded to nationally aired television commercials in December 2020. 3 The extent to which this campaign influenced public awareness of TED, to our knowledge, has yet to be described.Google Trends (Google) is an analytical tool that has been increasingly used in medical research to gauge public interests 5 and track disease outbreaks. 6 Similar applications of this tool are observed in the ophthalmology literature; Google Trends has been used to detect conjunctivitis epidemics 7 and evaluate public interest in cataract and refractive procedures. 8 In this study, we sought to examine the influence of DTCA on internet-based information-seeking behavior pertaining to TED. MethodsIn this descriptive cross-sectional study, data that were downloaded from Google Trends on October 17, 2021, were used to characterize internet-searching behavior. The institutional re-IMPORTANCE Given that direct-to-consumer advertising campaigns have the potential to both positively and negatively affect patient care, clinicians would benefit from being conscious of changes in public interest that result from such campaigns. OBJECTIVE To understand how direct-to-consumer advertising was associated with information-seeking behavior based on Google Trends. DESIGN, SETTING, AND PARTICIPANTSThis descriptive cross-sectional study used Google Trends to examine relative search volumes for thyroid eye disease (TED) from January 1, 2004, to June 30, 2021, in the US. The data for this cross-sectional study were downloaded on October 17, 2021. This study included internet search engine queries originating within the US, as determined by Google Trends. MAIN OUTCOMES AND MEASURESMonthly changes in relative search volumes for the terms "thyroid eye disease," "Tepezza," and "teprotumumab." RESULTS From January 1, 2004, through June 30, 2019, searches for TED demonstrated a stable baseline level with a mean (SD) relative search volume of 1.78 (1.25). Pathologic retinal neovascularization is a potentially blinding consequence seen in many common diseases including diabetic retinopathy, retinopathy of prematurity, and retinal vaso-occlusive diseases. This study investigates epithelial membrane protein 2 (EMP2) and its role as a possible modulator of angiogenesis in human retinal pigment epithelium (RPE) under hypoxic conditions. To study its effects, the RPE cell line ARPE-19 was genetically modified to either overexpress EMP2 or knock down its levels, and RNA sequencing and western blot analysis was performed to confirm the changes in expression at the RNA and protein level, respectively. Protein expression was evaluated under both normoxic conditions or hypoxic stress. Capillary tube formation assays with human umbilical vein endothelial cells (HUVEC) were used to evaluate functional responses. EMP2 expression was found to positively correlate with expression of pro-angiogenic factors HIF1α and VEGF at both mRNA and protein levels under hypoxic conditions. Mechanistically, EMP2 stabilized HIF1α expression through downregulation of von Hippel Lindau protein (pVHL). EMP2 mediated changes in ARPE-19 cells were also found to alter the secretion of a paracrine factor(s) in conditioned media that can regulate HUVEC migration and capillary tube formation in in vitro functional angiogenesis assays. This study identifies EMP2 as a potential mediator of angiogenesis in a human RPE cell line. EMP2 levels positively correlate with pro-angiogenic mediators HIF1α and VEGF, and mechanistically, EMP2 regulates HIF1α through downregulation of pVHL. This study supports further investigation of EMP2 as a promising novel target for therapeutic treatment of pathologic neovascularization in the retina.
Purpose: Determine whether prenatal maternal characteristics such as sociodemographic characteristics, comorbidities, or pregnancy complications affect retinopathy of prematurity (ROP) development.Methods: Medical records of 236 mother-infant dyads from our institution were reviewed, only including dyads in which infants were born at 30 weeks gestational age or earlier. The primary outcome measure was the risk of ROP (defined Stage 1 or greater in either eye) and its association with prenatal maternal variables.Results: Maternal Medicaid insurance, smoking during pregnancy, and chorioamnionitis were associated with an increased risk of ROP. For Medicaid insurance and chorioamnionitis, these risks were not appreciably altered by adjustment for potential confounders.Conclusion: These results suggest that several prenatal maternal factors may independently affect the risk of ROP in preterm infants. Validation of our findings could aid in the identification of infants at high risk for ROP based on prenatal clinical features.
