OPrograma Nacional de Imunizações (PNI), criado em 1973 e responsável pela organização da política nacional de vacinação da população brasileira, comemorará 40 anos no dia 18 de setembro de 2013. Ao longo de quatro décadas, o PNI consolidou-se como o coordenador de uma relevante intervenção de Saúde Pública de caráter universal, a vacinação, contribuindo sobremaneira para a redução da morbidade e mortalidade por doenças transmissíveis no Brasil.A criação do PNI possibilitou o fortalecimento do papel do Ministério da Saúde na organização e coordenação das ações de vacinação que já eram realizadas há várias décadas e haviam sido responsáveis pela erradicação da varíola, cujo último caso registrado no Brasil data de abril de 1971.1 O Programa é, ao mesmo tempo, herdeiro de experiências exitosas da Saúde Pública brasileira e protagonista de um novo momento, no qual a complexidade do quadro epidemiológico e o desenvolvimento de novas vacinas passaram a exigir uma mais adequada e inédita maneira de organização das ações de vacinação.Essa mudança foi fundamental para assegurar a uniformidade do calendário vacinal, a introdução sustentável de novas vacinas, a padronização técnica, e a adoção de estratégias inovadoras como a combinação de vacinação de rotina com campanhas de vacinação, que tiveram um papel essencial na eliminação da poliomielite e do sarampo, alcançadas no período de existência do PNI.A contribuição do PNI fez-se ainda mais relevante a partir da construção do Sistema Único de Saúde (SUS) no final dos anos 1980, dando início a um movimento de descentralização que colocou o município como o executor primário e direto das ações de saúde, entre elas as de vacinação. Nesse cenário, o PNI tem garantido a oferta de vacinas seguras e eficazes para todos os grupos populacionais que são alvo de ações de imunização, como crianças, adolescentes, adultos, idosos e indígenas.O PNI tem se modernizado continuamente, tanto para ofertar novos imunobiológicos custo-efetivos como para implementar e fortalecer novos mecanismos e estratégias que garantam e ampliem o acesso da população às vacinas preconizadas, especialmente dos grupos mais vulneráveis. Atualmente, o PNI disponibiliza 43 produtos, entre vacinas, soros e imunoglobulinas.O Programa também promove o desenvolvimento de estudos avaliativos do impacto das vacinas na morbimortalidade e realiza a vigilância de eventos adversos, complementando assim a extensa cadeia de garantia da qualidade dos imunobiológicos utilizados. Para tanto, o PNI conta com o importante apoio de instituições acadêmicas.
