Historically, the black Moroccan population is heterogeneous, originating from Arab and sub-Saharan immigration, and Amazighs, an indigenous population. This study, first of its kind, allows to determine the peculiarities of hypertension in this population, in particular the response to the different therapeutic classes, which makes it possible to compare the results found with those of the studies carried out on the sub-Saharan or American black population.
COVID-19 vaccines have reduced both lethality and hospitalization rates of the novel coronavirus disease. Nevertheless, multiple side effects have been reported in the literature, most often are harmless. We report two cases of acute myocarditis, hospitalized in the emergency department for chest pain occurring after the second dose of mRNA vaccine AstraZeneca. The SARS-Cov-2 infection was ruled out in both patients with a negative PCR obtained by nasal swabs and normal thoracic CT scans. Both patients had high levels of high-sensitive cardiac troponin I. Acute coronary syndromes were excluded with cardiac catheterization. Cardiac Magnetic resonance imaging (MRI) showed signs in favor of acute myocarditis. The evolution was favorable for both patients after being put on anti-inflammatory treatment. The universality and accumulation of reports concerning acute myocarditis following COVID vaccination, in the absence of any other diagnostic element that could explain the myocardial injury, establish a strong causal link, although the etiopathogenesis of such injury remains poorly elucidated.
The heart patients represent an unfavorable field for coronavirus disease 19 (COVID-19), and therefore, any regular follow-up consultation, not urgent, represents a great danger to catch up with this fatal disease. According to this study, we propose telemedicine as a comfortable, safe and efficient way to follow up our cardiac patients remotely.
Telemedicine is defined by the practice of medicine at a distance using means of information and communications technology. Given the importance of clinical examination, cardiology is not very well suitedto telemedicine. However, just like inall medical specialties, telemedicine facilitates and supplements clinical medicine. The Diagnosis Center of Cardiology in Rabat welcomes patients from different regions of Morocco. The longdistance travel, difficulty of access to care in rural Morocco, transportation problems, and delayed appointments all make follow-up of patients a very difficult task, therefore, many patients miss their consultation appointments. To avoid this problem, we followed our patients by social messaging platforms, thenjudged the effectiveness of this methoda year later. Materials and Methods:-This is a prospective, comparative analytic study of 200 patients underoral anticoagulant therapy based on vitamin K antagonists (VKA), followed in cardiology consultation, all of whom come from rural areas where access to care is difficult.
