A survey of PDE4 inhibitors reveals that some compounds trigger intracellular aggregation of PDE4A4 into accretion foci through association with the ubiquitin-binding scaffold protein p62 (SQSTM1). We show that this effect is driven by inhibitor occupancy of the catalytic pocket and stabilization of a "capped state" in which a sequence within the enzyme's upstream conserved region 2 (UCR2) module folds across the catalytic pocket. Only certain inhibitors cause PDE4A4 foci formation, and the structural features responsible for driving the process are defined. Switching to the UCR2-capped state induces conformational transition in the enzyme's regulatory N-terminal portion, facilitating protein association events responsible for reversible aggregate assembly. PDE4-selective inhibitors able to trigger relocalization of PDE4A4 into foci can therefore be expected to exert actions on cells that extend beyond simple inhibition of PDE4 catalytic activity and that may arise from reconfiguring the enzyme's protein association partnerships.

In the principal cells of the renal collecting duct, arginine vasopressin (AVP) stimulates the synthesis of cAMP, leading to signaling events that culminate in the phosphorylation of aquaporin-2 water channels and their redistribution from intracellular domains to the plasma membrane via vesicular trafficking. The molecular mechanisms that control aquaporin-2 trafficking and the consequent water reabsorption, however, are not completely understood. Here, we used a cell-based assay and automated immunofluorescence microscopy to screen 17,700 small molecules for inhibitors of the cAMP-dependent redistribution of aquaporin-2. This approach identified 17 inhibitors, including 4-acetyldiphyllin, a selective blocker of vacuolar H + -ATPase that increases the pH of intracellular vesicles and causes accumulation of aquaporin-2 in the Golgi compartment. Although 4-acetyldiphyllin did not inhibit forskolin-induced increases in cAMP formation and downstream activation of protein kinase A (PKA), it did prevent cAMP/PKAdependent phosphorylation at serine 256 of aquaporin-2, which triggers the redistribution to the plasma membrane. It did not, however, prevent cAMP-induced changes to the phosphorylation status at serines 261 or 269. Last, we identified the fungicide fluconazole as an inhibitor of cAMP-mediated redistribution of aquaporin-2, but its target in this pathway remains unknown. In conclusion, our screening approach provides a method to begin dissecting molecular mechanisms underlying AVP-mediated water reabsorption, evidenced by our identification of 4-acetyldiphyllin as a modulator of aquaporin-2 trafficking.

Zusammenfassung Zusammenfassung Neue Arzneimittel sind in Deutschland unmittelbar für die Patientinnen und Patienten verfügbar: Nach der Zulassung werden die Arzneimittel automatisch von der gesetzlichen Krankenversicherung (GKV) erstattet – zum vom pharmazeutischen Unternehmer frei festgelegten Preis. Dies steht im Gegensatz zum Vorgehen in vielen anderen europäischen Ländern mit weitergehenden Regulierungen des Markt- und damit Erstattungszuganges.Der frei festgelegte Marktpreis wird erst in Preisverhandlungen zum Erstattungsbetrag in der GKV auf Basis der vom Gemeinsamen Bundesausschuss (G-BA) beschlossenen Nutzenbewertung angepasst und gilt ab dem 13. Monat nach Marktzugang. Krankenkassen und Ärztinnen und Ärzte sind mit einer zunehmenden Zahl hochpreisiger Arzneimittel konfrontiert, die durch beschleunigte Zulassungsverfahren eine eingeschränkte Datenbasis für eine Bewertung ihres Zusatznutzens mitbringen. Gesetzliche Änderungen sind dringend geboten, um die hohe Qualität der Gesundheitsversorgung und deren Finanzierbarkeit zu sichern. Dabei können die Einführung eines Interimspreises statt des frei festgelegten Marktpreises, ein rückwirkend geltender Erstattungsbetrag, eine Stärkung des Wettbewerbs auch unter Patentarzneimitteln sowie der gezielte, qualitätsgesicherte Einsatz von Arzneimitteln mit eingeschränkter Datenlage nur in spezialisierten Behandlungseinrichtungen, die sich an der Generierung von Wirksamkeits- und Sicherheitsdaten beteiligen, geeignete Schritte sein. Zudem wäre eine Stärkung der europäischen Zusammenarbeit für angemessene Erstattungspreise neuer Arzneimittel anzustreben.
