La obesidad es una enfermedad crónica que se encuentra presente en todos los grupos poblacionales y con una tendencia a aumentar en función del tiempo. Las mayores prevalencias se dan en la población adulta seguida por el grupo escolar, pre-escolar y adolescente. Los que viven en áreas urbanas, las mujeres y los considerados no pobres son los más afectados. La obesidad ya está presente en todo el mundo y ha sido declarada la epidemia del siglo XXI. Su presencia se relaciona a enfermedades como la diabetes mellitus 2, la hipertensión arterial, las dislipidemias y algunos tipos de cánceres. En los adolescentes ya se encuentra complicaciones como la resistencia a la insulina y el síndrome metabólico. Los factores que condicionan los cambios en los estilos de vida (patrones dietarios y sedentarismo) están cada vez más vigentes y con una tendencia a una mayor presencia en el futuro, lo cual hace que el panorama de la obesidad se vea sombrío. Pese a todas las recomendaciones dadas por organismos internacionales, en particular la Organización Mundial de la Salud (OMS), y con las medidas que ha tomado el Estado, en especial la ley de Promoción de la alimentación saludable para niños, niñas y adolescentes, la prevalencia de la obesidad continúa aumentando. Es menester hacer tomar conciencia a la población que la obesidad es una enfermedad que ocasiona complicaciones que comprometen muy seriamente la salud. Para ello es necesario seguir las recomendaciones que tienden a orientarnos en una alimentación saludable y en una promoción de la actividad física. Comprometer a aquellas personas e instituciones que tengan que ver de alguna manera con los estilos de vida, que se sumen al esfuerzo dirigido a ofrecerle una buena salud a la población.
Introducción. La obesidad es un problema de salud pública; el objetivo del presente estudio fue determinar la prevalencia nacional y distribución del sobrepeso(S), obesidad(O) y obesidad abdominal (OA), en el Perú. Métodos. Se realizó un estudio observacional mediante el análisis de la Encuesta Nacional de Hogares 2012-2013 en el que se evaluaron a 20488 adultos. El diagnostico nutricional se determinó considerando el índice de masa corporal según la OMS: 18,5 a 24,9 normal, 25 a <30 sobrepeso, y ≥ a 30 kg/m2 obesidad. Asimismo, para la evaluación de OA se consideró la circunferencia de cintura ≥ 102 cm para varones y ≥ 88 cm para mujeres. Resultados. La prevalencia nacional de sobrepeso fue de 40,5% y de obesidad 19,7%, con un total de 60,2% de población adulta con exceso de peso. El 23,4% de O fueron mujeres y 15,7% varones; la OA en mujeres fue 51,2% y 14,8% en varones Los departamentos con mayor prevalencia de exceso de peso fueron Tacna (73,8%) e Ica (72,2%). Los departamentos con mayor prevalencia de OA fueron Lima, Ica y Lambayeque, con prevalencias superiores al 40%. Se presentó mayor prevalencia de O y OA en mayores de 40 años, en población que vive en la costa, a menos de 1000 msnm y que reside en áreas urbanas. Conclusiones. El 60,2% de población adulta peruana presentó exceso de peso (S y O), predominantemente en Tacna e Ica, incrementándose la prevalencia respecto a lo reportado previamente.

Su presencia a edades tempranas implica una obligación de identificar la aparición de complicaciones como resistencia a la insulina y diabetes mellitus (DM). Métodos. Estudio observacional, descriptivo y transversal. Participaron 1206 mujeres adolescentes. Se definió la obesidad con valores de IMC ≥ 95p según la OMS. Se determinó niveles séricos de insulina, glucosa y perfil lipídico. Se usó el Homeostasis Model of Assesmente Index (HOMA-I), mediante la ecuación de Matthews con el valor ≥ 3,16 para definir resistencia a la insulina (RI). Para las dislipidemias: hipercolesterolemia ≥ 200 mg/dL, bajo C-HDL ≤ 40 mg/dL, alto C-LDL ≥ 130 mg/dL e hipetrigliceridemia ≥ 130 mg/dL. A las adolescentes obesas con RI se les hizo una prueba de tolerancia oral a la glucosa (PTG): glicemia de 140 a 199 mg/dL intolerantes a la glucosa y ≥ 200 mg/dL diabéticas. Resultados. El 25,1% (303) de la población fue obesa; 246 adolescentes obesas participaron de la evaluación bioquímica, 28,1% (69) de ellas presentaron RI. En las adolescentes obesas con y sin RI, el promedio de las variables bioquímicas en las primeras fueron mayores, siendo estas diferencias significativas estadísticamente, salvo el C-HDL. Diferencias entre la prevalencia de dislipidemias fueron significativas a excepción del C-HDL. La RI presentó un OR de 10,9 (IC 5,6), 12,1 (IC 4,9-30,1), y 7,6 (IC 3-19,5) con la hipertrigliceridemia, hipercolesterolemia y C-LDL alto. La PTG mostró un 3,3% de intolerantes y ninguna diabética. Conclusiones. El 28,1% (69) de adolescentes obesas presentaron RI; ninguna participante del estudio presentó DM. La valoración del estado nutricional es indispensable por cuanto permite realizar un diagnóstico y en base a ello proponer un tratamiento. La medida internacionalmente aceptada es el índice de masa corporal (IMC) (peso sobre talla al cuadrado), tanto para los niños, adolescentes y adultos. Donde hay diferencias es en los patrones de referencia utilizados, y esto es necesario tomarlo muy en cuenta. Los estudios en mujeres gestantes y no gestantes, independientemente de la referencia que se use, reportan que el principal problema existente es el sobrepeso y la obesidad. En razón de este diagnóstico, el tratamiento es bajar de peso, reconociendo que alcanzar el objetivo es sumamente difícil dado el poco tiempo con que se cuenta y, por otro lado, la creencia que tiene la gestante que eso puede representar un riesgo para su gestación y su producto. Sin embargo, se sugiere que el tratamiento debe consistir en recomendaciones generales, como decirle que disminuya su ingesta de alimentos y que opten por una alimentación saludable y alguna actividad física.
