Background While fever is mostly self-limiting, antibiotic prescription rates for febrile children are high. Although every parent who receives a prescription visits a pharmacy, we have limited insight into pharmacy employees’ experiences with these parents. Pharmacy employees do however exert an important role in ensuring children receive correct dosages and in advising parents on administration of antibiotics. Objective To describe pharmacists’ and pharmacy assistants’ experiences with parents contacting a pharmacy for their febrile child, and to identify ways of improving medication management of these children. Setting Community pharmacies in the Netherlands. Method A qualitative study including 24 Dutch pharmacy employees was conducted, performing four focus group discussions among pharmacy employees. Analysis was based on constant comparative technique using open and axial coding. Main outcome measure Pharmacy employees’ experiences with parents contacting a pharmacy for their febrile child. Results Three categories were identified: (1) workload and general experience, (2) inconsistent information on antibiotic prescriptions, (3) improving communication and collaboration. Pharmacy employees experienced that dosing errors in antibiotic prescriptions occur frequently and doctors provide inconsistent information on prescriptions. Consequently, they have to contact doctors, resulting in a higher workload for both stakeholders. They believe this can be improved by providing the indication for antibiotics on prescriptions, especially when deviating from standard dosages. Conclusion Pharmacy employees experience a high amount of dosing errors in paediatric antibiotic prescriptions. Providing the indication for antibiotics in febrile children on prescriptions, especially when deviating from standard dosages, can potentially reduce dosage errors and miscommunication between doctors and pharmacy employees.
There is currently limited and inconclusive evidence that actual GP use of POCT in primary care patients with acute cardiopulmonary symptoms leads to more accurate diagnosis and affects clinical management. However, some studies show promising results, especially when a POCT is combined with a clinical decision rule.
Inspecteer de huid op erythema migrans, waarvan sprake is bij: 1) een centrifugaal uitbreidende rode-blauwrode macula/ ring > 5 cm zonder papulae, vesiculae, schilfering of infiltratie, ongeacht of een tekenbeet is opgemerkt; 2) een centrifugaal uitbreidende macula/ring > 5 cm mét papulae, vesiculae, schilfering of infiltratie na een tekenbeet ter plaatse. Controleer om te differentiëren tussen erythema migrans en een directe reactie op de tekenbeet na 1 week bij laesies < 5 cm om vast te stellen of het erytheem zich heeft uitgebreid.Serologisch onderzoek na een tekenbeet of bij erythema migrans is niet zinvol omdat de kans op fout-negatieven groot is en het niet differentieert tussen een actieve en een eerdere infectie.Bespreek met de patiënt wanneer de teek > 24 uur op de huid heeft gezeten én wanneer binnen 72 uur na verwijdering van de teek een antibioticum kan worden ingenomen, de optie tot profylaxe. Benadruk dat "watchful waiting" ook verantwoord is. Bespreek de voor-en nadelen en besluit in samenspraak met de patiënt. Geef als antibioticumprofylaxe: 1 dosis doxycycline 200 mg aan niet-zwangere vrouwen en kinderen > 8 jaar. Bij zwangerschap: 1 dosis azitromycine 500 mg en kinderen > 6 maanden-8 jaar: azitromycine 10 mg/kg. Behandel erythema migrans met 10 dagen doxycycline 2 dd 100 mg, 14 dagen amoxicilline 3 dd 500 mg of 5 dagen azitromycine 1 dd 500 mg. Geef kinderen < 9 jaar amoxicilline 50 mg/ kg/dg in 3 doses, 14 dagen (maximaal 3 dd 500 mg). Verwijs bij een vermoeden van latere manifestaties van lymeziekte. Serologie kan dan overwogen worden. Wat moet ik uitleggen? Instrueer de patiënt de huid drie maanden te controleren en contact op te nemen bij het ontstaan van erythema migrans en/of griepachtige verschijnselen, neurologische-en/of gewrichtsklachten. Leg bij profylaxe uit dat het risico op lymeziekte verminderd wordt maar dat het niet geheel verdwijnt. Raad aan om tijdens het tekenseizoen (maart tot november) in de natuur op het pad te blijven en bedekkende kleding te dragen. DEET kan overwogen worden. Adviseer huidcontrole na bos/duinwandelingen. Verwijs naar Thuisarts.nl en eventueel naar de RIVM tekenbeet-app. Raad patiënten www. tekenradar.nl aan als ze willen weten in welke gebieden van Nederland een hoge activiteit van teken te verwachten is. ▪ Huisartsen zien steeds vaker patiënten met vragen over (huiduitslag na) een tekenbeet. De laatste jaren is er veel discussie geweest over diagnostiek en beleid bij (mogelijke) lymeziekte. Hoe kun je de patiënt het best informeren? Wat is het juiste beleid na een tekenbeet? Wat moet ik weten? De teek, de Ixodes ricinus, behoort tot de arachnida en komt voor in duinen en bossen. In Nederland blijkt tot 29% van de teken besmet met Borrelia burgdorferi, de bacterie die lymeziekte kan veroorzaken. In 2009 zagen huisartsen 564 op 100.000 patiënten met een tekenbeet en 134 op 100.000 met erythema migrans. Naar schatting worden er jaarlijks 1 miljoen Nederlanders gebeten door een teek.
