Se analiza la epidemiología, diagnóstico, manejo y pronóstico de la Bacteriuria Asintomática en las pacientes que asistieron a control prenatal en el Instituto Materno Infantil de Bogotá y Clínica Hospital Fray Bartolomé de las Casas, desde el 1o de mayo de 1987 hasta el 30 de abril de 1988. La frecuencia de la Bacteriuria Asintomática en la mujer embarazada fue del 3%. El 80% de las pacientes estaban entre los 20 y 35 años. El 60% eran multíparas; la totalidad de raza blanca y mestiza y el 97% de nivel socioeconómico bajo y medio. El 33% tenían factores de riesgo como anemia, preeclampsia, enfermedad renal crónica y flujo genital. En el 64% se hizo el diagnóstico en el primero y segundo trimestre, siendo el E. Coli el germen más frecuente (73%) y el tratamiento se hizo de acuerdo con el antibiograma, teniendo en cuenta las drogas que se pueden utilizar durante el ambarazo, con una persistencia del 4% y una recidiva del 4.1% y ninguna paciente presentó pielonefritis. La morbilidad materna fue del 26%: Amenaza del parto pretérmino, preeclampsia y RPM. La morbilidad perinatal fue del 23%: Pretérmino y Retardo del crecimiento intrauterino.
Durante el período comprendido entre el primero de enero a 31 de diciembre de 1987, se realizó un estudio de casos controles en el Departamento de Ginecología y Obstetricia, de la Facultad de Medicina de la Universidad Nacional, en el Instituto Materno Infantil de Bogotá, con el fin de conocer el comportamiento del parto en las adolescentes frente a un grupo de mayor edad en iguales condiciones socio-económicas. Durante este período se atendieron 8.692 partos, de los cuales 728 fueron en adolescentes (8.37%).Para efectos de este estudio se eligieron al azar 200 adolescentes, entre 12 a 17 años y sus correspondientes controles, primigestantes de 20 y 24 años de edad.Se encontraron los siguientes resultados:En relación con el estado civil se encontró que la mayoría de población era soltera y sin unión estable, siendo la unión libre más frecuente en los controles. Los factores de riesgo estadísticamente mayores en las adolescentes fueron: Preeclampsia, anemia, estrechez pélvica, parto instrumentado, menor peso en el neonato.Se distribuyeron por igual los siguientes factores de riesgos: la infección de vías urinarias, la cual es alta en ambos grupos, el peso del neonato menor de 2.000 gr. en ambos grupos (4.5%), lo mismo que la talla de los neonatos y las malaformaciones congénitas.Riesgos como el estado del recién nacido, calificación de Apgar menor de 7 fue mayor en los controles, lo mismo que las complicaciones del puerperio inmediato.Además un mayor número de cesáreas, partos en pelvis, RPM, complicaciones del puerperio inmediato, sin que fueran estadísticamente significativos.
Se analizan los resultados de 50 casos del primer estudio prospectivo realizado en el país para el tratamiento de la Incontinencia Urinaria de Esfuerzo (IUE) en la mujer, mediante la Suspensión Endoscópica del Cuello Vesical (SECV) y clstostomía, desde abril de 1984 hasta septiembre de 1985.El 40% presentaba IUE tipo II y el 60% tipo III. El 94% tenía celes concomitante y el 30% antecedentes de cirugías ginecológicas. Se observó obesidad en el 48%. En el 97% de los casos se practicó SECV con corrección de celes y el promedio de tiempo quirúrgico fue de 49 minutos. Como complicación operatoria se anota la perforación vesical en el 6% de los casos, y en el postoperatorio granulomas de pared (6%), abscesos (2%) e infección de la colpoperineorrafia (2%). El drenaje por cistostomía duró en promedio 3.3. días, observándose infección urinaria en el 16%de los casos y retención urinaria posterior en el 4% Veintiséis (26) pacientes tuvieron seguimiento de 12 a 18 meses, con resultados exitosos en todas. Se presentaron dos fracasos antes de los seis meses, imputables a fallas en la técnica quirúrgica.Con los resultados obtenidos los autores consideran provisionalmente la SECV como la técnica ideal para el manejo de la IUE asociada con celes, obesidad y antecedentes de cirugías ginecológicas, en razón a la ubicación adecuada del cuello vesical, al menor tiempo quirúrgico y a la baja morbilidad. Se recomienda sistematizar el drenaje mediante cistostomía. Un seguimiento mayor de las pacientes permitirá obtener conclusiones definitivas.
Se analizaron 500 casos de I.U.E. en la mujer, mediante la Colpocistouretropexia suprapúbica, entre el 1 de julio de 1977 y el 30 de junio de 1986, en los Servicios de Ginecología y Urología del Instituto Materno Infantil y el Centro Hospitalario San Juan de Dios, y de Ginecología de la Clínica Fray Bartolomé de las Casas de Bogotá.La frecuencia de la Colpocistouretropexia fue de 3.8%. El 56.2% presentaba IUE de Grado II y el 43.8% Grado III. El 96% tenía celes concomitantes discriminados así: Grado I 34.8%, Grado II 54.4% y Grado III 6.8%. El 12.4% tenía antecedente de cirugía de Kelly y el l% de Colpocistouretropexia suprapúbica.Se practicaron los siguientes exámenes paraclínicos: Urocultivo: 100%, Citología vaginal: 93.7%, Citoscopia: 100%, Urografía: 40%, Urodinamia: 11.2%, Cistometría: 19.6%, Cistografía: 14%. Se practicaron los siguientes procedimientos quirúrgicos:Suprapúbicos: 35.6%, Suprapúbico + Colporrafia Posterior: 49.2%. Suprapúbico + Histerectomía Abdominal: 12%- Suprapúbico + Pomeroy 1.8%. Suprapúbico + Cirugía Vaginall.6%. El tiempo quirúrgico fue de 1 hora 45 minutos. En el 54% se utilizó la incisión de Pfannestiel. En el-Y,.8% se utilizó material no absorbible. En el 87.14% se utilizó drenes. En el 51.8% las intervenciones fueron realizadas por Residentes.Las complicaciones intra-operatorias fueron: Ruptura vesical 1.6% -Hematuria 8.4% -Hemorragia 5.2%. Las complicaciones post-operatorias fueron: Infección urinaria 16.8%-Infección de la herida 9%- Retención urinaria 16.8%-Dolor suprapúbico 11.14%. El promedio de permanencia de la sonda dude 4.8%. La estancia hospitalaria fue de 32.8%, a dos años 24.8% y de 3 a 5 años 31%. La eficacia fue del 96.4%. Con base en la experiencia durante 10 años en nuestros servicios, consideramos que la Colpocistouretropexia suprapúbica es un procedimiento eficiente en el manejo de la IUE en la mujer sin desconocer la morbilidad que conlleva.
La mujer embarazada, presenta cambios anatomofisiológicos que predisponen a la I.U.G., en sus diferentes modalidades: cistitis, bacteriuria asintomática y pielonefritis. El manejo de las dos primeras es similar, basándose en esquemas de duración variable con penicilinas, cefalosporinas, sulfonamidas y antisépticos urinarios, asegurando la eliminación de la bacteriuria hasta el final de la gestación, con control seriado de uroanálisis.En caso de piclonefritis, infección urinaria recurrente o anomalía anatomofuncional del tracto genito urinario se indica terapia supresora por lo que resta de la gestación.
