Znieczulenia porodów posiadają wieloletnią historię od epoki eteru i chloroformu z połowy XIX wieku do czasów współczesnych - metod kompleksowego podejścia do leczenia bólu porodowego z wykorzystaniem medycyny opartej na dowodach (EBM). Najnowsze techniki analgezji regionalnej obejmują niskodawkowe znieczulenie zewnątrzoponowe umożliwiające chodzenie pacjentek oraz kontrolowany przez pacjentkę wlew środka znieczulającego do przestrzeni ZO, postępy w farmakoterapii pozwoliły wykorzystać remifentanyl do kontrolowanej przez pacjentkę analgezji dożylnej (PCA), pojawiły się nowe leki miejscowo znieczulające i adiuwanty do stosowania w analgezji zewnątrzoponowej, takie jak Ropiwakaina, Levobupiwakaina, Sufentanyl, Klonidyna, Neostygmina. Zastosowanie podtlenku azotu w mieszaninie z tlenem i wziewnych środków anestetycznych w postaci waporyzatorów osobistych z metoksyfluranem (Penthrox) wydaje się obiecującą metodą łagodzenia bólów porodowych. Postęp technologiczny, taki jak wykorzystanie ultradźwięków do lokalizacji przestrzeni zewnątrzoponowej w trudnych przypadkach, minimalizuje nieudane znieczulenie zewnątrzoponowe. Wprowadzenie nowatorskich metod podawania leków, takich jak pompy do znieczulenia zewnątrzoponowego kontrolowanego przez pacjenta (PCEA) poprawiły ogólny wskaźnik zadowolenia matek i umożliwiły dostosowanie odpowiedniego schematu przeciwbólowego dla każdej rodzącej. Ostatnie badania randomizowane i analizy Cochrane’a wykazały, że związek znieczulenia zewnątrzoponowego ze zwiększonym odsetkiem porodów zakończonych cięciem cesarskim i długotrwałym bólem pleców pozostaje tylko mitem. Badania wykazały również, że nowsze, niskodawkowe schematy analgezji nie mają statystycznie istotnego wpływu na czas trwania porodu i początek karmienia piersią, a także, że zmniejszają one odsetek porodów instrumentalnych, poprawiając w ten sposób bezpieczeństwo matek i płodów.
Cesarean section is a obstetric procedure known since ancient times. It is mentioned in almost all ancient cultures. While until the 17th century it was performed mainly post mortem to retrieve an infant from the body of the deceased mother, today there are many indications for termination of pregnancy in this way. Literature provides references to earlier attempts to save the mother, but this happened only sporadically. The first record of a cc that both mother and child survived dates back to 1500. The development of science and medicine contributed to the dissemination and improvement of the methods used to extract a child by caesarean section. It was influenced by the development of operational, anesthetic and aseptic techniques. A milestone was the introduction of the Pfannenstiel – Kerr lower transverse incision method. Later, the Misgav-Ladach technique was introduced. Today, transverse incisions are most commonly used.
Pain is both a mental and a physical condition. It is considered by many experts to be a serious health problem or even a disease one. WHO describes it as a multidimensional phenomenon consisting of sensory, physiological, affective, spiritual and behavioral factors. Childbirth has always been associated with severe pain. The pain of childbirth has been known for centuries and is presented in every possible way as a very unpleasant sensation, not only physical but also mental. Over the years, efforts have been made to develop the best possible methods of relieving labor pain. These include non-pharmacological, such as: acupuncture, TENS or hypnosis, and pharmacological: administration of opioids and inhalation anesthetics, ending with regional analgesia techniques such as: epidural, subarachnoid anesthesia, etc. The article presents the history of the treatment of labor pain.
